Objective: to evaluate predictors on bad outcome of low-dose thrombolytic therapy with intravenous alteplase (0.6mg/kg) in acute ischemic strokes within 3 hours of onset. Methods: a prospective, observative study on 66 patients with acute ischemic stroke at Emergency Department, Bach Mai hospital from 1/2010 to 1/2012. Results: Factors of good outcomes were: Age above 70 (OR 3.0; 95 percent CI: 0.51-18.4; p=0.219), time from onset to admission above 70 minutes (OR 32; 95 percent CI: 1.23-83.46; p=0.037), glycemia above 10 mmol/l (OR 84; 95 percent CI: 0.76-325.1; p=0.017), NIHSS score above 16 (OR 0.43; CI: 0.05-3.9; p=0.45), middle cerebral artery occlusion M1 (OR 0,89; CI: 0.11-6.71; p=0.91), no recanalization (OR 38; CI: 4.27-34.48; p=0.00l). Condusion: The time from onset to admission above 70 minutes, glycemia above 10 mmol/l and no recanalization were predictors on bad outcome in patients treated with low-dose intravenous alteplase.
