Lower face defect caused by many reasons such as malignant and benign tumors, trauma or post radiotherapy. In consequence, such defects always reduce quality of life of the patient, and also is a challenge for the plastic and reconstructive surgeons. With the development of micro surgery and intensive studies of free flaps, the reconstruction of complicated lower face defect become much better. The authors present here the initial out come from 12 cases, including clinical features, technique and experience in using free osteocutaneous fibular flap for lower face defect reconstruction. With 81 percent good result, the improvement of patients' life have been proven.
