Xác định các sai sót liên quan đến thuốc trong điều trị nội trú tại bệnh viện đa khoa Hoàn Mỹ Minh Hải năm 2021. Đối tượng và phương pháp nghiên cứu: nghiên cứu mô tả cắt ngang tiến hành trên 442 hồ sơ bệnh án của bệnh nhân điều trị nội trú tại khoa Nhi, khoa Nội tổng hợp, khoa Tim mạch - Nội tiết, khoa Ngoại, khoa Sản tại bệnh viện đa khoa Hoàn Mỹ Minh Hải từ tháng 07/2021 đến tháng 9/2021. Số liệu được thu thập, xử lý bằng phần mềm Excel 2016 và SPSS 20.0. Kết quả: tỷ lệ sai sót liên quan đến thuốc là 4,07%. Các sai sót liên quan đến thuốc xảy ra nhiều nhất tại Khoa Nội tổng hợp (55,56%), kế đến là Khoa Tim mạch – nội tiết (27,78%). Sai sót liên quan đến thuốc ở giai đoạn chuẩn bị và thực hiện thuốc chiếm đa số với (33,33%), giai đoạn sao chép y lệnh (27,78%). Đối tượng thường xảy ra sai sót liên quan đến thuốc là điều dưỡng với 72,22%, dược sĩ (16,67%) và bác sĩ với (11,11%). Đối tượng phát hiện sai sót liên quan đến thuốc đa phần là dược sĩ (77,78%). Kết luận: hầu hết các giai đoạn trong quá trình sử dụng thuốc đều xảy ra sai sót liên quan đến thuốc. Việc phát hiện các sai sót này sẽ giúp nhân viên y tế đánh giá lại và tìm ra giải pháp để làm giảm sai sót.

Determining medication errors in inpatient in Hoan My Minh Hai general hospital in 2021. Materials and method: a descriptive cross-sectional study was conducted on 442 medical records of inpatients at the Pediatrics Department, Internal Medicine Department, Cardiology – Endocrinology Department, General Surgery Department, Obstetrics and Gynecology Department in Hoan My Minh Hai general hospital from July 2021 to September 2021. Data were collected and analyzed by using Excel 2016 and SPSS 20.0 software. Results: the ratio of medication errors was 4.07%. Medication errors occurred most in the Internal Medicine Department (55.56%), followed by the Cardiology – Endocrinology Department (27.78%) Medication errors occurred in the medication preparation and administration stage (33.33%), followed by the transcription stage (27.78%). The person who often made medication errors was nurse with 72.22%, followed by pharmacist with 16.67% and doctor with 11.11%. Pharmacist is the person who detected medication errors most (77.78%). Conclusion: most stages of drug using process occured medication errors. Detection of medication errors will help medical staff to re-evaluate and find solutions to reduce medication errors.
