Khảo sát hình ảnh nội soi và mô bệnh học của polyp đại trực tràng (ĐTT) kích thước trên 10mm. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 84 bệnh nhân tại Trung tâm tiêu hóa gan mật - Bệnh viện Bạch Mai từ 01/2017 - 12/2021. Thực hiện nội soi toàn bộ đại tràng, chọn polyp kích thước lớn nhất và > 10mm để mô tả đặc điểm và tiến hành cắt polyp, lấy bệnh phẩm làm mô bệnh học theo tiêu chí WHO 2010. Kết quả: 89,2% polyp ở đại tràng đoạn gần với 82,1% polyp có cuống và 20,2% polyp kích thước > 20mm. Polyp u tuyến chiếm tỷ lệ cao 84,5%, chủ yếu là polyp u tuyến ống 91,6% với 100% có loạn sản ở các mức độ khác nhau, trong đó 18,3% loạn sản độ cao. Chưa xác định được mối liên quan giữa kích thước, hình dạng polyp với mô bệnh học của polyp u tuyến ĐTT > 10mm. Kết luận: polyp ĐTT kích thước > 10mm chủ yếu polyp u tuyến, ít có thành phần nhung mao và không liên quan đến đặc điểm vị trí, kích thước, hình dạng trên nội soi.

To study endoscopic images and histopathology of colorectal polyps over 10mm in size. Subjects and methods: A descriptive cross-sectional study on 104 patients at Gastroenterohepatology Center - Bach Mai Hospital from January 2017 to December 2021. Colonoscopy was performed to evaluate characteristics of polyps over 10mm in size and performing polypectomy to evaluate histopathology according to WHO 2010 criteria. Result: 89.2% of polyps in proximal colon with stalk is the most common shape (accounting for 82.1%). There are 20,2% polyp over 20mm in size. Adenomatous polyps accounted for a high rate of 84.5%, mainly tubular adenoma (91.6%) with 100% having dysplasia of various degrees, of which 18.3% were high grade dysplasia. Size is the only relationship between polyp and the degree of dysplasia on histopathology of adenomatous polyps. Conclusion: Colorectal polyps over 10mm in size are mainly adenomatous ones, uncommon villous component and not relationship with site, shape, size of polyp in colonic endoscopy.
