Phẫu thuật nội soi đang ngày càng thay thế nhiều phẫu thuật mổ mở kinh điển. Cắt khối tá tuỵ nội soi toàn bộ cũng đã dần được thực hiện tại các trung tâm ngoại khoa trên Thế giới và Việt Nam, tuy nhiên số lượng chưa nhiều. Chúng tôi thông báo trường hợp đầu tiên cắt khối tá tuỵ nội soi toàn bộ được thực hiện tại Bệnh viện Bạch Mai và xem lại y văn về chỉ định, kết quả điều trị. Bệnh nhân nữ, 64 tuổi, chẩn đoán ung thư bóng Vater (T3N0M0), Phẫu thuật cắt khối tá tuỵ nội soi hoàn toàn. Thời gian mổ 540 phút, mất máu 150ml, sau mổ có chảy máu ổ bụng ngày thứ 4 điều trị nội khoa ổn định, không có các biến chứng khác. Bệnh nhân ra viện ngày thứ 23. Giải phẫu bệnh sau mổ: ung thư biểu mô tuyến, di căn 4/32 hạch nạo vét (T3bN2M0). Trường hợp này đã cho thấy cắt khối tá tuỵ nội soi hoàn toàn có thể thực hiện được ở bệnh nhân ung thư vùng tá tràng đầu tuỵ có thể cắt bỏ, chưa xâm lấn mạch máu và cho kết quả tốt.

Laparoscopic surgery is increasingly replacing classic open surgery. Total laparoscopic pancreaticoduodenectomy (TLPD) has also gradually performed at many centers in the world and Vietnam, but the number is still modest. We hereby report the first successful total laparoscopic pancreaticoduodenectomy case performed at Bach Mai Hospital and review the medical literature on indications and results. This is a 64-year-old female patient, diagnosed with ampulla of Vater cancer (T3N0M0) underwent total laparoscopic unbloc-pancreaticoduodenectomy. Surgical time was 540 minutes, with blood loss of 150ml. Patient had postoperative intra-abdominal bleeding on day 4, was stabilized with medical treatment. The patient was discharged on the 23rd day with no other complication. Postoperative pathological result: adenocarcinoma, metastasis in 4/32 lymph nodes (T3bN2M0). This case shows that a total laparoscopic pancreaticoduodenectomy can be performed safely and has good oncological result with resectable periambullary tumors.
