Đối chiếu chẩn đoán tế bào học theo hệ thống phân loại Bethesda với kết quả mô bệnh học ở bệnh nhân ung thư biểu mô tuyến giáp (UTBMTG) được phẫu thuật tại Bệnh viện Trung ương Quân đội 108 (TƯQĐ 108). 529 trường hợp được chẩn đoán tế bào học trước phẫu thuật và có kết quả mô bệnh học sau mổ tại Bệnh viện TƯQĐ 108 là UTBMTG từ tháng 01/2020 đến tháng 4/2021. Phân loại tế bào học theo hệ thống phân loại tế bào tuyến giáp Bethesda 2018 và phân loại mô bệnh học tuyến giáp theo Tổ chức Y tế Thế giới (WHO) năm 2017. UTBMTG gặp ở nữ/nam là 4/1, u ở vị trí thuỳ phải gặp nhiều nhất (42,9%) và hiếm gặp ở eo tuyến giáp (6,4%); kích thước u trung bình 1,1± 0,9cm; 66,2% kích thước < 1cm. Chẩn đoán UTBMTG ở nhóm Bethesda V là cao nhất, tiếp đến là nhóm VI tương ứng là 47,1% và 29,6%. Nhóm Bethesda I có kích thước u < 0,5cm chiếm cao nhất (55,6%), không có trường hợp nào với u kích thước > 1cm, nhóm Bethesda II ≥ 2 nhân chiếm 63,6%. Chẩn đoán UTBMTG thể nhú gặp nhiều nhất ở nhóm V (48,5%) và tiếp đến là nhóm VI (30,1%), trong khi UTBMTG thể nang gặp ở nhóm IV là cao nhất (53,8%), không gặp trường hợp thể nang nào thuộc nhóm II và VI. Phân loại nhóm tế bào u kích thước nhỏ có xu hướng ác tính cao hơn so với nhóm u có kích thước lớn hơn (p=0,001). Chẩn đoán tế bào học UTBMTG gặp chủ yếu trong nhóm V và VI và phần lớn là thuộc thể nhú, thể nang gặp trong nhóm IV, phân loại nhóm tế bào lành tính và ác tính có sự khác biệt về kích thước khối u.

To compare the cytological diagnosis according to the 2018 Bethesda classification with the histopathology of thyroid carcinoma at 108 Military Central Hospital. 529 cases were diagnosed with preoperative cytology and post-operative histopathological results at 108 Military Central Hospital as thyroid carcinoma f-rom January 2020 to April 2021. The cytological diagnosis was according to the 2018 Bethesda system for reporting thyroid cytopathology. The histopathological classification was according to the 2017 World Health Organization classification of thyroid neoplasms. The rate of thyroid carcinoma in females/males was 4/1. The tumor was common in the right lobe (42.9%) and rare in the isthmus (6.4%), with the average tumor size being 1.1 ± 0.9cm and 66.2% of the cases were less than 1cm. The diagnosis of thyroid carcinoma was mainly found in the groups of Bethesda V and VI, with 47.1% and 29.6% of the cases, respectively. The Bethesda I group with tumor size < 0.5cm accounted for the highest proportion, with 55.6% of the tumors, and the Bethesda II group with ≥ 2 neoplasms had a rate of 63.6%. The diagnosis of papillary thyroid carcinoma was more common in group V (48.5%), followed by group VI (30.1%), while the number of follicular thyroid carcinoma in group IV was the highest (53.8%). None of the follicular carcinoma belongs to groups II and VI. Cytological classification with small-size tumors tend to be higher in a malignant group compared with tumor groups with larger sizes. The cytological diagnosis of thyroid carcinoma was found mainly in groups V and VI, with the papillary being the most common type. The follicular thyroid carcinoma, on the other hand, was commonly found in group IV. Cytological classification for thyroid gland in a benign and malignant group has statistically significant differentiation in tumor sizes.
