Xuất huyết tiêu hoá dưới là một hội chứng thường gặp ở trẻ em, với biểu hiện lâm sàng đi ngoài phân có máu, phân đen hoặc máu ẩn trong phân. Mục tiêu: Xác định nguyên nhân và mô tả đặc điểm lâm sàng xuất huyết tiêu hoá dưới ở trẻ em tại Bệnh viện Nhi Trung ương. Đối tượng và phương pháp: nghiên cứu tiến cứu, mô tả cắt ngang 170 ca bệnh được chẩn đoán xuất huyết tiêu hoá dưới tại Bệnh viện Nhi Trung ương. Kết quả: Tuổi trung bình là 5,5 ± 4,6 tuổi (40 ngày đến 17 tuổi), tỷ lệ nam/nữ là 1,4/1. Triệu chứng đi ngoài phân máu tươi chiếm tỷ lệ cao nhất 61,2%. Các triệu chứng đi kèm phổ biến gồm tiêu chảy kéo dài (31,2%), thiếu máu (31,2%), đau bụng (27,6%). Có 93,1% bệnh nhân phát hiện được tổn thương trên nội soi đại tràng trong đó tổn thương phổ biến nhất là polyp. Các nguyên nhân xuất huyết tiêu hoá thường gặp là polyp (60,6%), tiêu chảy kéo dài nhiễm khuẩn (14,6%), bệnh ruột viêm (9,4%), viêm túi thừa Meckel (4,7%) với tỷ lệ khác nhau theo lứa tuổi bệnh nhân. Kết luận: đi ngoài phân lẫn máu tươi là triệu chứng thường gặp nhất ở bệnh nhân xuất huyết tiêu hoá dưới. Nội soi đại tràng là phương pháp có giá trị để chẩn đoán xác định nguyên nhân xuất huyết tiêu hóa dưới.

Lower gastrointestinal bleeding is a common syndrome in children, with clinical manifestations include hematochezia, melena or occult hidden or unseen. Aim: to describe clinical and endoscopy characteristics of lower gastrointestinal bleeding in children at Vietnam National Children's Hospital. Materials and methods: a case series included 170 patients diagnosed with lower gastrointestinal bleeding at the National Hospital of Pediatrics. Results: The mean age at onset of symptoms was 5.5 ± 4.6 years old (40 days to 17 years old). Morbidity rate of male/female was 1.4:1. Hematochezia was seen in 61.2%. The accompanying symptoms were prolonged diarrhea (31.2%), anemia (31.2%), abdominal pain (27.6%). Lesions on colonoscopy were found in 93.1% of patients in which 61.2% was polyp. Common causes of lower gastrointestinal bleeding were are polyps (60.6%), infectious diarrhea (14.6%), inflammatory bowel disease (9.4%), Meckel’s diverticulum (4.7%). Conclusion: hematochezia was the most common symptoms of lower gastrointestinal bleeding. Colonoscopy is an important diagnostic method to confirm the causes of lower gastrointestinal bleeding.
