Mô tả đặc điểm lâm sàng, căn nguyên và đánh giá kết quả điều trị các nhiễm trùng vùng đầu mặt cổ ở các bệnh nhi được điều trị nội trú tại khoa Răng Hàm Mặt, Bệnh viện Nhi trung ương. Phương pháp Nghiên cứu mô tả hồi cứu 147 ca nhiễm trùng vùng đầu mặt cổ, so sánh theo nhóm nhiễm trùng da – mô mềm và nhiễm trùng có nguyên nhân răng miệng, tuổi từ 01 tháng tuổi đến 11 tuổi điều trị tại Khoa Răng Hàm Mặt. Kết quả Nghiên cứu gồm 147 bệnh nhân 101 bệnh nhân nhiễm trùng da – mô mềm và 46 bệnh nhân nhiễm trùng do răng. Kết quả nuôi cấy vi khuẩn (bệnh phẩm mủ) dương tính ở 75/81 mẫu thuộc nhóm nhiễm trùng da – mô mềm, 2/4 mẫu ở nhóm nhiễm trùng do răng. Căn nguyên vi khuẩn thường gặp ở nhóm nhiễm trùng da – mô mềm gồm Staphylococcus aureus - chiếm 62/75 (82,7%) mẫu dương tính (54 mẫu là chủng kháng methilicillin). Phương pháp điều trị bao gồm nội khoa cho 62 bệnh nhân ở thể viêm tấy (42 bệnh nhân thuộc nhóm nhiễm trùng do răng) và nội khoa kết hợp dẫn lưu mủ ở 85 bệnh nhân ở thể áp xe (82 bệnh nhân nhiễm trùng da – mô mềm). Kết quả 46/147 bệnh nhân đáp ứng tốt với điều trị, 1 bệnh nhân được chuyển sang Trung tâm các Bệnh nhiệt đới do có biểu hiện nhiễm trùng huyết; có 2 trường hợp tái phát sau khi ra viện. Kết luận Nhiễm trùng phần mềm đầu mặt cổ trẻ em thường gặp là nhiễm trùng da – mô mềm hoặc nhiễm trùng do răng; phương pháp điều trị bao gồm nội khoa, có thể kết hợp dẫn lưu mủ; căn nguyên vi khuẩn thường gặp ở nhiễm trùng da – mô mềm là Staphylococcus aureus.

To describe clinical features, etiology and evaluate treatment outcome of inpatients having head - face and neck infections in Odonto-Maxillofacial Surgery Department, Vietnam National Children’s Hospital. Methods Retrospective study on 147 patients, from 1 month to 11 years old, compared between two groups skin and soft tissue infection an odontogenic infection. Results Study included 101 skin - soft tissue infection patients and 46 odontogenic infection patients. The culture result was positive on 75/81 samples of skin - soft tissue infection group and 2/4 samples of odontogenic infection group. The most common bacterial agent was Staphylococcus aureus, counting for 62/75 positive cases (54 cases were the Methicillin resistance Staphylococcus aureus). Solely antibiotic therapy was applied for 62 patients (42 odontogenic cases), and 85 patients (81 non-tooth infection patients) were treated with antibiotic combining drainage. During treatment, 146/147 patients responded well to treatment, 1 patient was transferred to the Tropical Diseases Center due to septicemia. There were 2 cases relapsed after being discharged from the hospital.
