Trầm cảm là bệnh lý đi kèm thường gặp ở người bệnh viêm khớp dạng thấp, trầm cảm làm các triệu chứng viêm khớp dạng thấp nặng hơn, biến chứng xảy ra nhiều hơn, tăng tỷ lệ tử vong và tự sát trên người bệnh, từ đó gây ra gánh nặng lớn đến người bệnh, gia đình và xã hội. Có nhiều nghiên cứu về trầm cảm ở người bệnh viêm khớp dạng thấp trên thế giới, tuy nhiên tại Việt Nam chưa có nhiều nghiên cứu về vấn đề này. Do đó chúng tôi thực hiện nghiên cứu: “Trầm cảm và một số yếu tố liên quan ở người bệnh viêm khớp dạng thấp tại Khoa Cơ xương khớp – Bệnh viện Bạch Mai” với mục tiêu: “Xác định tỉ lệ trầm cảm và một số yếu tố liên quan ở người bệnh viêm khớp dạng thấp điều trị nội trú tại khoa Cơ xương khớp Bệnh viện Bạch Mai”. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang 79 người bệnh viêm khớp dạng thấp điều trị nội trú tại khoa Cơ xương khớp Bệnh viện Bạch Mai từ tháng 6/2019 – 6/2020. Kết quả: Đối tượng nghiên cứu chủ yếu là nữ (83,5%), độ tuổi trung bình 60,9 ± 9,3, nơi sinh nông thôn và thành thị tương đương nhau, trình độ học vấn trung học cơ sở 40,5%. Có tới 51,9% người bệnh có trầm cảm theo thang đánh giá trầm cảm Hamilton. Sự khác biệt có ý nghĩa thống kê về số khớp đau, số khớp sưng, nồng độ CRP, chỉ số mức độ hoạt động bệnh trên 28 khớp tính theo CRP (DAS 28 – CRP) ở nhóm người bệnh trầm cảm và không trầm cảm. Những người bệnh không thể lao động, sinh hoạt, kiếm tiền có tỉ lệ trầm cảm cao hơn những người bệnh còn duy trì khả năng lao động, sinh hoạt (p < 0,05).

Depressive disorder is a common comorbid disease in the rheumatoid arthritis disease (RA), which will adversely make the RA symptoms worse, complications increased, mortality and suicide rates increased which is a heavy burden for the patient, their family and society. there has been a lot of research about depression disorder in rheumatoid arthritis patient in the world but it is still lack of report in Viet Nam. Therefore, we carry out research named “Depression and some related fators in rheumatoid arthritis patients in rheumatology department – Bach Mai hospital”. Subjects and research methods: Descriptive cross-sectional study of 79 rheumatoid arthritis inpatients at the Department of Rheumatology in Bach Mai Hospital from June 2019 to June 2020. Results: studying subjects are mainly female (83.5%), the average age is 60.9 ± 9.3, where urban is equivalent to rural, 40.5% is secondary education. There are 51.9% of patients who are depressive according to the Hamilton Depression Scale (HAM - D). Significantly different is the number of joint pain and swollen joints, CRP index, disease activity index in 28 joint following CRP (DAS 28- CRP) in depressed patient and non-depressed patient. The patients who are unable to work, do activities of daily living or make money have a higher depression rate than the others who still remain their workability and activities of daily living with p<0.05.
