Trong nhiễm khuẩn nặng, phản ứng miễn dịch mạnh mẽ và các cytokine, các chất trung gian gây viêm có thể kích hoạt quá trình đông máu dẫn đến tình trạng rối loạn đông máu trong toàn cơ thể. Trong một số nghiên cứu gần đây, fibrinogen đã được báo cáo như một dấu ấn sinh học của tình trạng này. Mục tiêu: phân tích nồng độ fibrinogen huyết tương ở trẻ nhiễm khuẩn nặng nhập PICU và làm sáng tỏ mối quan hệ giữa nồng độ fibrinogen và tỷ lệ tử vong ở bệnh nhi bị nhiễm khuẩn nặng. Phương pháp: Các bệnh nhi được chẩn đoán nhiễm khuẩn nặng theo IPSCC 2005, không có các bệnh lý mãn tính, nhập PICU từ tháng 6 năm 2021 đến hết tháng 7 năm 2022 được đưa vào nghiên cứu. Các thông số lâm sàng và xét nghiệm cũng như kết quả điều trị tại bệnh viện đã được thu thập và phân tích. Kết quả: Tổng số 68 bệnh nhi đã được đưa vào nghiên cứu. Tỷ lệ tử vong chung tại bệnh viện là 22,1% (15/68). Nồng độ fibrinogen thấp hơn đáng kể ở những bệnh nhi tử vong so với những bệnh nhi người sống sót. Hơn nữa, diện tích dưới đường cong (ROC) đối với fibrinogen trong dự đoán tử vong tại bệnh viện là 0,780 (KTC 95%: 0,711-0,850). Kết luận: Fibrinogen là một dấu ấn sinh học có giá trị tiên lượng cho nhiễm khuẩn nặng ở trẻ em. Nồng độ fibrinogen thấp khi nhập PICU có liên quan chặt chẽ đến gia tăng nguy cơ tử vong ở bệnh nhi nhiễm trùng huyết.

In severe sepsis, immune response, cytokines and inflammatory mediators can active coagulation leading to coagulation disorders all over the body. In recent studies, Fibrinogen has been reported as a biomarker of coagulopathy. Objectives: to analyze plasma fibrinogen levels in severe sepsis children admitted to PICU and elucidate the relationship between fibrinogen levels and mortality in pediatric patients with severe sepsis. Methods: Pediatric patients diagnosed with severe sepsis according to IPSCC 2005, without chronic diseases, admitted to the PICU from June 2021 to the end of July 2022 were included in the study. Clinical and laboratory parameters as well as treatment outcomes at the hospital were collected and analyzed. Results: A total of 69 patients were included in the study. The overall in-hospital mortality rate was 21.7% (15/69). Fibrinogen plasma level were significantly lower in non-survivors than in survivors. Furthermore, the area under the curve (ROC) for fibrinogen in predicting hospital mortality was 0.780 (95% CI: 0.711-0.850). Conclusion: Fibrinogen is a valuable prognostic biomarker for severe bacterial infections in children. Hypofibrinogen on admission to PICU are strongly associated with an increased risk of mortality in severe sepsis.
