Khảo sát các đặc điểm của phương pháp đo áp lực hậu môn trực tràng (ALHMTT) và mối liên quan với chức năng đại tiện của bệnh nhân (BN) sau phẫu thuật (PT) bệnh Hirschsprung. Phương pháp: Nghiên cứu cắt ngang mô tả được thực hiện tại bệnh viện Nhi Đồng 2. Các BN được PT Hirschsprung tại Bệnh viện Nhi Đồng 2 từ tháng 1 năm 2015 đến tháng 1 năm 2020 được liên lạc mời tham gia nghiên cứu và thực hiện đo ALHMTT. Các đặc điểm đo ALHMTT, lâm sàng và chức năng đại tiện của BN tham gia nghiên cứu được ghi nhận và phân tích. Nhóm dân số tham chiếu cũng được thực hiện để so sánh về chức năng đại tiện và kết quả đo ALHMTT với nhóm dân số PT. Kết quả: Tổng cộng 95 bệnh nhân PT và 95 tham chiếu đã tham gia với 68% nam giới; và tuổi trung bình lần lượt là 6,6 ± 2,2 tuổi và 7,2 ± 2,9 tuổi. Trong nhóm PT, tỷ lệ són phân và tiêu bón lần lượt là 25,2% và 13,7%, nhóm tham chiếu lần lượt là 2,1% và 4,2 (P <0,05). Giá trị trung bình của áp lực HM lúc nghỉ của bệnh nhân PT thấp hơn có ý nghĩa thống kê so với nhóm tham chiếu (53,2 ± 16,1 mmHg so với 62,2 ± 14,0 mmHg; p <0,05). Trong nhóm PT, BN són phân nặng có áp lực HM lúc nghỉ trrung bình thấp hơn có ý nghĩa thống kê so với các trường hợp không són phân (46,0 ± 10,6 mmHg so với 55,6 ± 16,9 mmHg; p <0,05). Khi thực hiện nghiệm pháp cảm giác, thể tích dung nạp tối đa (Vmax) của nhóm PT thấp hơn không có ý nghĩa thống kê so với nhóm tham chiếu (P> 0,05). Tuy nhiên, trong nhóm PT, những BN són phân có Vmax trung bình thấp hơn có ý nghĩa thống kê so với nhóm không són phân (p <0,05). Kết luận: Đo ALHMTT là phương pháp khách quan cung cấp thông tin hữu ích giúp hướng dẫn phương pháp xử trí thích hợp hơn ở những BN RLĐT nặng sau PT bệnh Hirschsprung.

To investigate anorectal manometry (AM) findings and functional outcomes of patients operated on for Hirschsprung’s disease (HD). Methods: A cross-sectional study was conducted at Children’s Hospital 2. Patients operated on for HD from January 2015 to January 2020 were reviewed and recontacted to investigate their bowel function and to undergo an AM. We studied their clinical characteristics and bowel function as a long-term outcome and its relationship with manometric findings. A reference-based population was recruited for comparisons regarding bowel function and AM results with the operated patients. Results: In total, 95 operated patients and 95 references were enrolled, with 68% males, and the mean ages at follow-up were 6.6 ± 2.2 years and 7.2 ± 2.9 years, respectively. The rates of fecal incontinence and constipation were 25.2% and 13.7%, respectively, for the cases; the references were 2.1% and 4.2, respectively (P<0.05). The anal resting pressure means of operated patients were significantly lower than those of the references (53.2 ± 16.1 mmHg versus 62.2 ± 14.0 mmHg; p<0.05). Within the cases, the incontinents had shown significantly lower anal resting pressure than the continents (46.0 ± 10.6 mmHg versus 55.6 ± 16.9 mmHg; p<0.05). When the sensation test, the mean value of maximum tolerated volume was non-significant different from that of the references (P>0.05). However, within the operated group, the incontinents had shown a significantly lower mean value of Vmax than the continents (p<0.05). Conclusions: AM is an objective method providing helpful information that could guide to a more adapted management in patients with defecation disorders after HD operation.
