Kết quả phẫu thuật u màng não góc cầu - tiểu não bằng đường mổ sau xoang xích ma

U màng não góc cầu-tiểu não là u thường gặp thứ hai ở vùng góc cầu-tiểu não, chiếm 6 - 15% u vùng này. Phẫu thuật lấy toàn bộ u màng não góc cầu-tiểu não là thách thức lớn. Mục tiêu của nghiên cứu là đánh giá kết quả phẫu thuật u màng não góc cầu-tiểu não. Phương pháp: nghiên cứu tiến cứu trên 36 bệnh nhân u màng não góc cầu-tiểu não được phẫu thuật bằng đường mổ sau xoang xích ma từ 8/2020 đến 8/2022 tại Bệnh viện Việt Đức. Kết quả: Tuổi trung binh là 56,1 và tỉ lệ nữ: nam là 3,5:1. Đa số là u màng não độ I chiếm 91,7%, độ II là 8,7%. Các triệu chứng lâm sàng thường gặp nhất là đau đầu (69,4%), chóng mặt (44,4%), rối loạn thăng bằng (36,1%), giảm thính lực (19,4%). Phần lớn u (91%) bám ở mặt sau xương đá. Tỉ lệ lấy u toàn bộ đạt 69% và không có tử vong sau mổ. Tỉ lệ biến chứng dưới 10%. Khả năng bảo tồn chức năng dây thần kinh mặt đạt 72%. Kết luận: Đường mổ sau xoang xích ma là đường mổ an toàn để lấy u màng não góc cầu-tiểu não. Đường mổ này có thể giúp lấy u tối đa mà vẫn bảo tồn chức năng thần kinh và tỉ lệ biến chứng sau mổ thấp.
