Nhịn đói qua đêm trước phẫu thuật nhằm tránh biến chứng hít sặc phổi, tuy nhiên gây ra khó chịu cho người bệnh. Hiệp hội Tăng Cường Phục Hồi Sau Phẫu Thuật (ERAS) và Hiệp Hội Gây Mê Châu Âu (ESA) đã khẳng định sử dụng carbohydrate đường uống trước phẫu thuật 2 giờ là an toàn và cải thiện cảm giác khó chịu liên quan phẫu thuật của người bệnh. Chúng tôi tiến hành thử nghiệm lâm sàng ngẫu nhiên có đối chứng trên 40 bệnh nhân phẫu thuật cắt túi mật nội soi. Nhóm can thiệp được uống 200ml dung dịch maltodextrin 12,5% trước phẫu thuật 2 - 4 giờ. Nhóm chứng nhịn ăn uống từ 22 giờ ngày hôm trước phẫu thuật. Đánh giá tỉ lệ hít sặc phổi, thể dịch dịch tồn dư dạ dày trước phẫu thuật, cảm giác đói, khát, khô miệng, đau tại các thời điểm: lúc nhập viện, trước phẫu thuật, sau phẫu thuật. Nghiên cứu cho thấy không có trường hợp hít sặc phổi nào, thể tích dịch tồn dư dạ dày giữa 2 nhóm không có sự khác biệt có ý nghĩa thống kê (p = 0,1682). Giá trị trung bình điểm đói, khát, khô miệng trước phẫu thuật ở nhóm can thiệp thấp hơn có ý nghĩa thống kê so với nhóm chứng (p < 0,01). Điểm cảm giác đói, khát, khô miệng, đau sau phẫu thuật ở nhóm can thiệp thấp hơn nhóm chứng không có ý nghĩa thống kê (p > 0,05). Kết luận: Bổ sung 200ml dung dịch maltodextrin 12,5 % đường uống 2 - 4 giờ trước phẫu thuật cắt túi mật nội soi là an toàn và giúp cải thiện cảm giác đói, khát, khô miệng trước phẫu thuật.

The purpose of fasting before anesthesia is to reduce the risk of regurgitation and aspiration. Prolonged fasting for many hours prior to surgery could lead to patient discomfort. According to the Enhance recovery after surgery® Society (ERAS) and Europe Society of Anesthesia (ESA), oral pre-operative carbohydrate loading 2 hours before surgery is safe and reduce the discomfort associated with surgery. We design a randomised controlled trial, a total 40 patients with laparoscopic cholecystectomy in Hanoi Medical University Hospital. The intervention group receives maltodextrin 12.5% (200ml) 2 to 4 hours before surgery. The control group fasted from 22 hours according to the standard protocol. Discomfort scale was used to score of hunger, thirst, mouth dryness, and pain. Gastric residual volume preoperative was measured by nasogastric sonde. There was no case of lung aspiration in the intervention group. There was no difference of gastric residual volume in two groups (p = 0.1682). The average of pre-operative score discomfort was lower in the intervention group than the control group (p < 0.001). The average of post-operative score discomfort was lower in the intervention group than the control group (p > 0.05). Conclusion: pre-operative oral maltodextrin 12,5% is safe and effective on reducing patient pre-operative discomfort.
