Khảo sát tình hình nhiễm Candida máu, sự nhạy cảm của các thuốc kháng nấm, phân tích sử dụng thuốc kháng nấm và xác định một số yếu tố liên quan đến đáp ứng điều trị nhiễm Candida máu tại bệnh viện Nhân Dân Gia Định. Đối tượng và phương pháp nghiên cứu: Nghiên cứu cắt ngang mô tả được thực hiện trên 35 hồ sơ bệnh án của người bệnh nhiễm Candida máu giai đoạn 2019 – 2020. Tính hợp lý trong sử dụng thuốc kháng nấm được đánh giá dựa trên Hướng dẫn điều trị nhiễm nấm xâm lấn của Hiệp hội bệnh truyền nhiễm Mỹ cập nhật năm 2016. Kết quả: Tuổi trung bình của người bệnh nhiễm Candida máu là 67,6±14,7 tuổi. Bệnh mạn tính mắc kèm thường gặp ở người bệnh nhiễm Candida máu là tăng huyết áp (65,7%) và đái tháo đường (57,1%). Các yếu tố nguy cơ được khảo sát đều chiếm tỷ lệ cao (thấp nhất là nhiễm Candida ở vị trí khác với 34,3%). C. tropicalis chiếm tỷ lệ cao nhất với 40,0%. Phần lớn các loài Candida vẫn còn nhạy cảm cao với các thuốc kháng nấm như amphotericin B, caspofungin, micafungin và voriconazol hơn so với fluconazol. Khi xét riêng về loài, kết quả kháng nấm đồ cho thấy C. albicans vẫn còn nhạy 100% với tất cả các thuốc kháng nấm trong khi các loài C. non-albicans đã xuất hiện nhiều chủng đề kháng. Kết quả phân tích hồi quy logistic cho thấy yếu tố chỉ định cấy máu theo dõi sau điều trị liên hệ có ý nghĩa thống kê với đáp ứng điều trị (OR = 6,250, khoảng tin cậy 95% = 1,399 – 27,925, p = 0,016). Kết luận: Loài C. non-abicans chiếm tỷ lệ cao với nhiều chủng kháng các thuốc kháng nấm đặc biệt là fluconazol. Cấy máu theo dõi sau khi sử dụng thuốc kháng nấm có thể góp phần giúp gia tăng hiệu quả điều trị nhiễm Candida máu.

This study aimed to characterize patients with candidemia, antifungal susceptibility, to analyze the use of antifungal drugs and identify factors associated with candidemia treatment outcome at Nhan dan Gia Dinh Hospital. Materials and methods: A retrospective descriptive cross-sectional study based on medical records of 35 patients with candidemia between 2019 and 2020. The appropriateness of drug administration was assessed according to guideline for the management of candidiasis 2016 update by the Infectious Diseases Society of America. Result: The mean age of the patients was 67,6±14,7 years. Hypertension (65,7%) and diabetes (57,1%) were the most common comorbidities in patients with candidemia. Risk factors which were evaluated have a high proportion (the smallest was multifocal candida species colonization with 34,3%). C. tropicalis was the most popular species with 40,0%. Amphotericin B, caspofungin, micafungin and voriconazol were more susceptible with antifungals than fluconazol. C. non-albicans was less susceptible than C. albicans specifically. The logistic regression analysis suggested that follow-up blood cultures was associate with the treatment outcome (OR = 6,250, CI 95% = 1,399 – 27,925, p = 0,016). Conclusion: C. non-albicans was the most common species which was less susceptible with many antifungals, fluconazol especially. Followup blood cultures can improve the treatment outcome.
