Ung thư hắc tố da, Di căn hạch, Mô bệnh học, Yếu tố liên quan

Nghiên cứu đặc điểm lâm sàng, mô bệnh học và liên quan giữa di căn hạch với một số yếu tố mô bệnh học của ung thư hắc tố da tại viện K. 60 bệnh nhân ung thư huyết tố da giai đoạn II, III được điều trị tại viện K. Thiết kế mô tả hồi cứu và tiến cứu. Hay gặp tuổi 40 - 70, nữ/nam 1,2. Xuất hiện nhiều chi dưới 70%, u màu đen 65%, nốt ruồi to 38,3%, thể lan tràn nông 73,3%, di căn hạch 28,3%, bề dày u > 2 mm 86,7%,, loét 23,3%, Clark IV,V 78,4%. Giai đoạn phát triển thẳng 90%, xâm nhập mạch 18.3%, vệ tinh 31.7%. nhóm thưa thớt lympho xâm nhập 61,1%. Di căn hạch thể nốt 75%. Di căn hạch nhóm T3, T4 cao hơn T1, T2, 34% so với 7,7%, nhóm có loét cao hơn nhóm không loét, 42,8% so với 23,9%, ở Clark IV - V cao hơn Clark I - II - III, 34% so với 7,7%, nhóm có giai đoạn phát triển thẳng đứng cao hơn nhóm không có thẳng đứng, 31,5% so với 0%, nhóm có xâm nhập mạch cao hơn nhóm không có, 90,9% so với 14,3%, nhóm có nhân vệ tinh cao hơn nhóm không có, 47,4% so với 19,5%. Di căn hạch 0% nhóm lympho bào xâm nhập dày đặc, 20% nhóm thưa thớt, 52,6% nhóm không có.Kết luận thường gặp ở tuổi 40 - 70, nữ/nam 1,2, bệnh hay gặp chi dưới, màu đen, nốt ruồi to, thể lan tràn nông, bề dày u > 2 mm, Clark IV, V, có giai đoạn phát triển thẳng đứng, di căn hạch thể nốt, nhóm thưa thớt lympho bào xâm nhập, u có loét 23,3%, di căn hạch 28,3%, xâm nhập mạch 18,3%, nhân vệ tinh 31,7%. Di căn hạch tăng khi độ dầy u tăng, loét u, Clark tăng, có giai đoạn phát triển, có xâm nhập mạch ,có nhân vệ tinh và lympho xâm nhập u giảm.
