Introduction: The confirmed diagnosis of a testicular and epididymitis tuberculous case is a challenge for clinicians because testicular tuberculosis is similar to that scrotal neoplasm. This study reports a case of tuberculous testicles and epididymitis in a 41-year-old male patient with large swollen testicles. Case report: A 41-year-old male patient had history of pulmonary tuberculous treatment in previous11 years before hospital admission. Patient detected testicles were swollen and ached many years ago. Patient visited to a hospital in Danang City for requesting examining. This hospital had suspected this patient had tuberculous testicles and sent him to the Danang Hospital for conducting histological examining test. The result of histological test confirmed that he was exactly had tubercular testicles and epididymitis with the histological lesions presenting as nodule showing caseating granuloma with Langhan's giant. Patient was transferred to Danang Hospital for Tuberculosis and Lung diseases for treatment suitably with speciality on tuberculosis and lung diseases. Conclusion: Although a man with testicular swelling, pain is very easy to think of testicular cancer, but the diagnosis should make careful, meticulous to avoid mistakenly removed the testes of TB cases crystal full misdiagnosed as testicular cancer. The clinicians need to know to make sure testicular tuberculosis cured if diagnosed property and treated with anti-TB drugs available.
