Mô tả tình trạng quá tải sắt ở bệnh nhân Thalassemia bằng ferritin huyết thanh. 2. Đánh giá hiệu quả thải sắt bằng thuốc Deferasirox ở bệnh nhân Thalassemia. Phương pháp: Thiết kế nghiên cứu mô tả, hồi cứu. Nghiên cứu trên 84 bệnh nhân Thalassemia từ 6 tuổi trở lên, có quá tải sắt điều trị tại Viện Huyết học-Truyền máu TW từ 1/2016 đến 12/2020 được điều trị bằng Deferasirox trong thời gian nghiên cứu. Chỉ số nghiên cứu: Ferritin huyết thanh, Hb khi BN vào viện, lượng thuốc dùng, thể tích máu truyền, chức năng gan, thận ít nhất 3 tháng một lần. Kết quả: có 84 bệnh nhân nghiên cứu với tuổi trung bình là 14,7 tuổi (7-46); nhóm bệnh nhân phụ thuộc truyền máu (TDT) là 76,2%. Bệnh nhân β- Thalassemia và β-thalassemia/HbE chiếm 91,7%. Tình trạng quá tải sắt của BN tại thời điểm bắt đầu nghiên cứu: 39,3% quá tải sắt mức độ nặng, 50% quá tải sắt trung bình và 10,7% quá tải sắt nhẹ. Sau 5 năm điều trị thải sắt bằng Deferasirox với liều lượng trung bình là 15,1 mg/kg/ngày và lượng máu được truyền trung bình là 65,7 ml/kg/ngày. Kết quả: 87% BN có giảm nồng độ Ferritin, 13% BN bị tăng Ferritin. Tỷ lệ bệnh nhân bị quá tải sắt mức độ nặng giảm từ 39,3% xuống 21,4%, quá tải sắt mức độ trung bình giảm từ 50% xuống 36,9%, có 9,5% BN đạt được nồng độ ferritin trong giới hạn bình thường. Kết luận: Deferasirox có hiệu quả thải sắt trong việc giảm ferritin huyết thanh ở bệnh nhân Thalassemia có quá tải sắt và duy trì truyền máu định kỳ.

To determine iron overload in thalassemia patients by serum ferritin. To evaluate long term iron chelation efficiency in thalassemia patients treated with Deferasirox. Methods: Descriptive, retrospective study. We conducted a study on 84 thalassemia patients aged from 6 years with iron overload treated at the National Institute of Hematology and Blood Transfusion from January 2016 to December 2020 who were treated by Deferasirox during the study period. These patients were clinically examined, evaluated monthly serum ferritin, Hb, transfused blood volume, drug dose, drug side effects, liver and kidney function throughout the study period. Results: 84 patients in this study with the average age of 14.7 years old (7-46); Blood transfusion dependent patients contributed 76.2% and β- Thalassemia and β- Thalassemia/ HbE accounted for 91,7%. Iron overload status of patients at the beginning of the study was 39.3% severe iron overload, 50% moderate iron overload and 10.7% mild iron overload. After 5 years of iron chelation therapy with Deferasirox with an average dose of 15.1 mg/kg/day and a mean blood transfusion of 65.7 ml/kg/day, the results: Ferritin levels was decreased in 87% of patients and increased in 13% of patients. The porportion of patients with severe iron overload decreased from 39,3% to 21,4%, moderate iron overload decreased from 50% to 36,9%, and 9,5% of patients had no iron overload. The average ferritin concentration decreased gradually over the years, with the figure of the transfusion-dependent Thalassemia (TDT) group reduced to 1215 ng/ml while that of non transfusion-dependent thalassemia (NTDT) group reduced to 884 ng/ml. The average of both groups decreased to 1136 ng/ml. Conclusion: Deferasirox is effective in reducing serum ferritin in thalassemia patients who suffer from iron overload as well as undergo regularly blood transfusion.
