Nghiên cứu nhằm mục tiêu đánh giá tác dụng giảm đau và các tác dụng không mong muốn của phương pháp phong bế thần kinh thẹn tiêm một lần dưới hướng dẫn siêu âm sau phẫu thuật cắt trĩ . 20 bệnh nhân phẫu thuật cắt trĩ theo chương trình được được giảm đau sau mổ bằng phương pháp phong bế thần kinh thẹn tiêm một lần dưới hướng dẫn siêu âm tại khoa Gây mê hồi sức và Chống đau – Bệnh viện Đại học Y Hà Nội từ tháng 1 đến tháng 4 năm 2021. Thời gian thực hiện kĩ thuật, vùng phong bế cảm giác, điểm đau VAS khi nghỉ và khi vận động, mức độ hài lòng của bệnh nhân và một số tác dụng không mong muốn được ghi lại trong 24 giờ sau mổ. Thời gian thực hiện kĩ thuật trung bình là 12,1 ± 1,6 (phút). 89,5% người bệnh phong bế được vùng tam giác chậu phía sau; 10,5% người bệnh có phong bế toàn bộ tầng sinh môn; không có ai bị phong bế vùng tam giác trước. Độ sâu trung bình từ mặt da cho đến vị trí TK thẹn là 5,1 ± 0,9 (cm). Điểm VAS trung bình khi nghỉ đều < 3 và khi vận động đều xấp xỉ 4 ở tất cả các thời điểm. 90,0% bệnh nhân có mức độ hài lòng và rất hài lòng. Nghiên cứu của chúng tôi cho thấy phong bế thần kinh thẹn tiêm một lần dưới hướng dẫn siêu âm có hiệu quả giảm đau tốt sau mổ cho các phẫu thuật cắt trĩ.

Our study aimed to assess the analgesic efficacy and the side effects of single shot pudendal nerve block guided by ultrasound at patients with hemorrhoid surgery. 20 patients with hemorrhoid surgery were relieved of postoperative pain by single shot pudendal nerve block guided by ultrasound from January 2021 to April 2021 at Departement of Anesthesia, Critical Care & Pain Management, Hanoi Medical University Hospital. Sensory blockade, VAS points at rest and during movement, patient satisfaction and the side effects were recorded in 24 hours postoperatively. The average time of procedure is 12,1 ± 1,6 (min). The average depth from the skin surface to the pudendal nerve position was 5.1 ± 0.9 (cm). 89,5 % patients were the blockade of anterior triangle (urogenital triangle), 10,5 % total pelvic floor. 90 % of patients have very satisfied and satisfied levels. Mean VAS scores at rest were below 3 points and during exercise were approximately 4 points during 24 hours postoperative. Our study is showed that single shot pudendal nerve block guided by ultrasound is an efficient for postoperative pain management at patients with hemorrhoid surgery.
