Bệnh nhân cao tuổi trải qua phẫu thuật ngày càng nhiều, tuy nhiên đây cũng là nhóm bệnh nhân có sự không đồng nhất về chức năng và hoạt động sống cơ bản. Và suy yếu được xác định là yếu tố nguy cơ chính làm tăng nguy cơ tử vong, biến chứng phẫu thuật, thời gian nằm viện và hoạt động chức năng. Mục tiêu: Đánh giá tình trạng suy yếu và mối liên quan giữa suy yếu ở bệnh nhân cao tuổi với kết quả điều trị sau phẫu thuật. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang 322 bệnh nhân trên 60 tuổi có trải qua phẫu thuật tại khoa ngoại tổng hợp, khoa gan mật tuỵ, phẫu thuật lồng ngực - Bệnh viện Bạch Mai chúng tôi khảo sát tình trạng suy yếu theo tiêu chuẩn Fried. Chúng tôi theo dõi bệnh nhân 30 ngày sau phẫu thuật, ghi nhận các kết cục gồm: biến chứng, tử vong trong vòng 30 ngày sau phẫu thuật. Kết quả: Trong 322 bệnh nhân cao tuổi được phẫu thuật theo chương trình, có 110 bệnh nhân suy yếu trước phẫu thuật theo tiêu chuẩn Fried, chiếm tỉ lệ là 34,16%. Trong vòng 30 ngày sau phẫu thuật, có 101 bệnh nhân có biến chứng/ tử vong, chiếm tỉ lệ là 31,4%. Tỉ lệ biến chứng/ tử vong ở nhóm bệnh nhân suy yếu là 57,8%; lớn hơn so với nhóm bệnh nhân không suy yếu (17,2%) và sự khác biệt này có ý nghĩa thống kê với p <0,001. Kết luận: Suy yếu là hội chứng phổ biến trên bệnh nhân cao tuổi phẫu thuật và làm tăng đáng kể biến chứng sau phẫu thuật. Nên đánh giá suy yếu một cách thường quy.

Older adults are a highly heterogeneous group with variable health and functional life courses. Beside, Older patients are undergoing surgery in increasing numbers and frailty is a key risk factor associated with higher rates of complications and mortality, longer hospital stays, and functional impairment. Objectives: To assess the frailty and the association between fraitly and Clinical outcomes in older patients undergoing surgery. Methods: cross-sectional study, 322 patients 60 years old or older were undergoing surgery in the Department general surgery,Hepato-biliary-Pancreatic Department,thoracic surgery Department at Bach Mai Hospital, we surveyed the weakened state according to Fried's criteria.We recorded the postoperative outcomes, defined complications and death, occurring within 30 days after surgery. Results: Of the 322 older patients undergoing elective surgery, 110 patients had fraitly, accounting for 34,16%. Within 30 days after surgery, 101 patients had complications and mortality, accounting for 31,4%. The rate of postoperative outcomes was significantly higher in frail group (57,8%) as compared to the non-frail group (17,2%), p <0.001. Conclusions: Frailty is a frequent syndrome in the perioperative setting and significantly increased complicative postoperation. So a routine frailty assessment is implemented.
