U xương dạng xương là khối u lành tính của hệ xương có đặc điểm một ổ tổn thương dạng xương được bao bọc bởi viền xơ xương và đặc xương xung quanh. U xương dạng xương hay gặp ở đầu các xương dài, chiếm 10% u xương lành tính và 1% u tại cột sống, hiếm gặp ở cột sống ngực. U xương dạng xương tại cột sống thường biểu hiện cơn đau tại chỗ, đặc biệt về đêm, co cứng cơ và có thể gây vẹo cột sống. Chẩn đoán dựa vào đặc điểm lâm sàng kết hợp CT-scanner, MRI và khẳng định bằng giải phẫu bệnh. Có nhiều phương pháp điều trị u xương dạng xương như điều trị bảo tồn, can thiệp qua da dưới hướng dẫn cắt lớp vi tính hay phẫu thuật cắt bỏ u. Việc lựa chọn phương pháp điều trị phù hợp cho u xương dạng xương tại cột sống phụ thuộc vào đặc điểm từng ca bệnh. Chúng tôi thông báo một trường hợp lâm sàng u xương dạng xương thân đốt sống ngực T2 ở bệnh nhân 17 tuổi được phẫu thuật lấy bỏ khối u, cố định cột sống lối sau qua đó xem xét lại y văn về căn bệnh này.

Osteoid osteoma (OO) is a benign bony tumor characterized by presence of a nidus of osteoid vascular bone with dense sclerotic bone in its periphery. Osteoid osteoma which usually localize within long bones, comprises 10% of all benign bone tumors, 1% of all spinal tumors, and infrequently located in thoracic spine. A spinal osteoid osteoma usually presents as dull back pain localized around level of lesion that is worse at night, paraspinal muscle contracture maybe causing postural scoliosis. The definitive diagnosis is based on the clinical symptoms associated with CT-scanner, MRI and confirmed by postoperative histopathology. There are several methods used so as to treat osteoid osteoma such as conservative management, percutaneous intervention using CT-guidance or tumor resection surgery. However, the appropriate treatments for osteoid osteoma in spine depend on particular cases. We report an osteoid osteoma in T2 vertebral spine in 17-year-old patient who underwent thoracic pedicle screw fixation with T2 laminectomy and tumor resection via a posterior approach thereby reviewing the literature on this disease.
