Aims: application of osteocutaneous fibular free flaps in the treatment of lower face through and through defect. Methods: 18 patients with complex lower face defects were operated using osteocutaneous fibular free flaps including 6 chimeric flaps. Results: 17/18 (94.5 percent) flaps survived and long time follow showed excellent outcome in 77 percent. Conclusion: Osteocuntaneous fibular free flap is a suitable material for lower face reconstruction, the high excellent result brings much better quality of life for the patients.
