Di căn màng não tủy - chùm đuôi ngựa gây hội chứng đuôi ngựa (Cauda Equina Syndrome: CES) ở bệnh nhân có khối u ác tính là hiếm gặp và có tiên lượng rất xấu. Theo y văn, tổn thương di căn màng não tủy - chùm đuôi ngựa phần lớn gặp ở bệnh nhân ung thư phổi (50%), ung thư vú (11%), ung thư đại trực tràng (3%), ung thư thận (10%), u ác tính (8%) và u lympho (4%). Tổn thương di căn chùm đuôi ngựa có thể gặp ở 1,65% bệnh nhân ung thư phổi nguyên phát. Sử dụng cộng hưởng từ có tiêm thuốc đối quang từ và 18F-FDG PET/CT là các phương pháp chẩn đoán hiệu quả. Việc phát hiện sớm, chính xác tổn thương di căn màng não tủy - chùm đuôi ngựa cho phép định hướng điều trị kịp thời. Trong bài viết này, chúng tôi thông báo một trường hợp bệnh nhân ung thư phổi không tế bào nhỏ di căn màng não tủy - chùm đuôi ngựa gây hội chứng đuôi ngựa được phát hiện bằng cộng hưởng từ có tiêm thuốc đối quang từ và 18F-FDG PET/CT tại Bệnh viện Trung ương Quân đội 108.

Cauda Equina Syndrome (CES) in patients with malignancies is rare and has a very poor prognosis. According to the literature, leptomeningeal - cauda equina metastases are mostly seen in patients with lung cancer (50%), breast cancer (11%), colorectal cancer (3%), kidney cancer (10%), melanoma (8%) and lymphoma (4%). Metastatic cauda equina may occur in 1.65% of patients with primary lung cancer. Using contrast-enhanced MRI and 18F-FDG PET/CT are effective diagnostic methods. The early, accurate detection of metastatic lesions of meninges and spinal cord - cauda equina allows timely treatment orientation. In this article, we report a case of non-small cell lung cancer with meningeal metastases - cauda equina causing cauda equina syndrome detected by contrast-enhanced MRI and 18F-FDG. PET/CT at 108 Military Central Hospital.
