Tổn thương thần kinh VII sau chấn thương vùng mặt nếu không được điều trị kịp thời sẽ ảnh hưởng đến chức năng, thẩm mỹ xấu, tác động đến tâm lý, sinh hoạt và chất lượng cuộc sống của người bệnh. Mục tiêu nghiên cứu: đánh giá kết quả phục hồi tình trạng liệt mặt sau phẫu thuật phục hồi thần kinh VII ngoại biên sau chấn thương vùng mặt. Đối tượng và phương pháp nghiên cứu: mô tả loạt ca lâm sàng bệnh nhân bị liệt mặt sau chấn thương vùng mặt được phẫu thuật nối hoặc ghép đoạn thần kinh mặt ngoại biên từ 01/2019 đến 12/2020 tại Bệnh viện Răng Hàm Mặt Trung Ương Thành phố Hồ Chí Minh. Kết quả: tất cả 21 trường hợp chấn thương trong vòng 8 tuần được phẫu thuật phục hồi thần kinh VII ngoại biên. 8/21 trường hợp được nối tận – tận thần kinh bị đứt, 9/21 trường hợp được ghép đoạn thần kinh bị đứt bằng thần kinh bì bắp chân ngoài (thần kinh Sural) và 4/21 trường hợp kết hợp nối tận – tận và ghép đoạn thần kinh bị đứt. 06 tháng sau phẫu thuật, tình trạng liệt mặt của bệnh nhân được cải thiện trên 20/21 bệnh nhân. Tuy nhiên có 3/21 trường hợp có xuất hiện tình trạng đồng vận sau phẫu thuật. Kết luận: Phẫu thuật nối - ghép đoạn thần kinh cho các trường hợp tổn thương thần kinh VII sau chấn thương sẽ giúp phục hồi dẫn truyền thần kinh và cải thiện tình trạng liệt mặt của bệnh nhân

Damage to the facial nerve after facial trauma, if not treated in time, will affect function, bad aesthetics, affect psychology, daily life, and quality of life of patients. Objectives: To evaluate the outcome of surgical repair of the facial nerve injury in the patients with facial trauma. Methods: cases study of patients suffering facial paralysis with facial trauma underwent end to end neurorrhaphy or using nerve graft from January 1, 2019, to December 12, 2020, at the Central Hospital of Odonto-Stomatology Hochiminh City. Results: 21 patients with traumatic facial nerve injury were included in our study. All cases underwent peripheral facial nerve repair within 8 weeks after the facial trauma. End to end neurorhaphy were performed in 8 patients (38%); Sural nerve graft primary were used in 9 patients (42.8%); using the Sural nerve as the nerve graft, combination of end to end anastomosis and nerve grafting were performed in 4 patients (19.2%). Facial nerve function was improved in 20 patients (95%) including complete restoration in 5 patients and partial in 15 patients. There were 3 patients (14,2%) had synectics facial nerve after the surgery. Conclusions: End-to-end anastomosis and nerve grafting of the injured nerve VII after trauma will help restore facial nerve conduction and improve the patient's facial paralysis.
