Purpose: To compare visual outcomes between wavefront-guided LASIK (WL) and conventional LASIK (CL). Methods: In this randomized prospective study, myopic and/or astigmatism eyes were treated by a Technolas 217 Z10b platform. Primary outcome measures were uncorrected (UCVA) and corrected (BCVA) visual acuities and manifest refraction. Secondary outcome measures were higher-order aberrations. Results: The WL group comprised 54 eyes and the CL group, 54 eyes. At 12 months, the mean UCVA was -0.019 log MAR '0.038 and -0.025 ' 0.053 log MAR in WL group and CL group, respectively (p=0.481). In both groups, 100 percent eyes achieved a UCVA of 20/20 or better in WL group and 96.8 percent eyes in CL group; 100 percent of eyes in both groups were within 0.50 diopter of emmetropia. The mean postoperative total HOA root mean square and spherical aberration was 0.518 - 0.163um and 0.355 - 0.101 um in the WL group and 0.628 - 0.191 um and 0.450 0.151 in the LASIK group, respectively (p0.05). Conclusions: Wavefront-guided LASIK and conventional LASIK had similar efficacy, predictability, safety; however, wavefront-guided LASIK induced statistically fewer HOAs than conventional LASIK at 12 months.
