Objective: To evaluate the effect of spinal anesthesia by combination of levobupivacine and fentanyl for laparoscopic appendectomy. Subject and method: A prospective study on 40 patients underwent spinal anesthesia by combination of levobupivacaine 0.18mg/kg with 30mcg fentanyl (total dose of levobupivacaine was s 12mg) for laparoscopic appendectomy. Result: The onset time of sense block was 7.25 +/- 1.42 minutes; the onset time of motor block at the first degree was 5.56 +/- 0.88 minutes; the duration of analgesia was 166.5 +/- 47.25 minutes; the duration of motor block at the first degree was 82.5 +/- 26.25 minutes, excellent level of anesthesia was 92.5 percent. Heart rate and mean blood pressure after spinal anesthesia were significantly lower than before spinal anesthesia (p0.05) but still within normal limits. Respiratory rate and EtC02 after CO2 insufflation were significantly higher than before CO2 insufflation (p0.05) but still within normal limits. Sp02 changed within normal limits during the surgery. Unwanted side effects were hypotension 5 percent, bradycardia 5 percent, shoulder pain 20 percent; pruritus 10 percent, and shivering 2.5 percent. Conclusion: Spinal anesthesia by combination of levobupivacaine and fentanyl was anesthetically sufficient for laparoscopic appendectomy.
