Nhận xét kết quả bước đầu áp dụng phẫu thuật bóc nhân qua niệu đạo với dao điện lưỡng cực trong điều trị u lành tuyến tiền liệt có thể tích lớn. Đối tượng và phương pháp: nghiên cứu tiến cứu mô tả hàng loạt ca bệnh được thực hiện trên các trường hợp u lành tuyến tiền liệt có chỉ định điều trị ngoại khoa với thể tích tuyến tiền liệt từ 60 – 120ml, được phẫu thuật bóc nhân qua niệu đạo bằng dao lưỡng cực, tại Bệnh viện Trung ương Quân đội 108, từ tháng 8/2021 đến tháng 2/2022. Kết quả: 12 bệnh nhân tuổi trung bình 68,1 và với thể tích tuyến tiền liệt 81,6 ml; thời gian bóc nhân trung bình là 66,2 phút; thời gian xay mô tuyến 33,7 phút; không xuất hiện các dấu hiệu của hội chứng nội soi trong mổ; không truyền máu trong mổ, không trường hợp nào chảy máu sau mổ; không có sự khác biệt có ý nghĩa về nồng độ Natri trong máu trước và sau mổ; thể tích tuyến tiền liệt không thấy có liên quan đến thời gian mổ cũng như sự mất máu trong mổ. Kết luận: Phẫu thuật bóc nhân qua niệu đạo bằng dao điện lưỡng cực an toàn đối với u tuyến tiền liệt có thể tích lớn.

To evaluate the initial result of bipolar transurethral enucleation of the prostate in the treatment of large-volume benign prostatic hyperplasia. Patients and methods: a prospective case-serial study was performed on patients with benign prostatic hyperplasia who had prostatic volume in the range of 60 to 120 ml. All the patients were undergone bipolar transurethral enucleation of the prostate in 108 Military Central Hospital, from August 2021 to February 2022. Results: There were 12 patients with 81.6 ml of the prostate on average. The mean age was 68.1 y-o. The duration of enucleation and morcellation were 66.2 mins and 33.7 mins, respectively. There was no sign of intra- or postoperative endoscopic syndrome. None of the patients needed a blood transfusion. The difference between pre- and postoperative sodium concentrations was not statistically. The prostate volume might not be related to the procedure time as well as blood loss. Conclusion: Bipolar transurethral enucleation of the prostate is safe in the surgical treatment of large volume benign prostatic hyperplasia.
