Trầm cảm là rối loạn tâm thần thường gặp nhất ở người bệnh chấn thương tuỷ sống. Trầm cảm tác động đến nhiều khía cạnh như làm suy giảm chất lượng cuộc sống, tập luyện và phục hồi kém hiệu quả, tăng nguy cơ tự sát, ảnh hưởng nặng nề tới quá trình phục hồi và tái hoà nhập xã hội của người bệnh. Mục tiêu nghiên cứu: Mô tả đặc điểm lâm sàng rối loạn trầm cảm ở người bệnh chấn thương tuỷ sống. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang 107 người bệnh chấn thương tuỷ sống điều trị nội trú tại Trung Tâm Phục Hồi Chức Năng bệnh viện Bạch Mai và Khoa Cột Sống Ít Xâm Lấn bệnh viện Châm Cứu Trung Ương từ tháng 08/2020 đến tháng 08/2021. Kết quả: Đối tượng nghiên cứu chủ yếu là nam giới (87,9%); độ tuổi trung bình 43,25 ± 13,74; nơi sinh sống chủ yếu ở nông thôn (65,4%); trình độ học vấn trung học cơ sở (38,3%). Có 32,7% người bệnh có rối loạn trầm cảm theo ICD10, trong đó trầm cảm nhẹ chiếm 18,7%; triệu chứng khởi phát hay gặp nhất là buồn chán, bi quan (40%); trong các triệu chứng đặc trưng của trầm cảm, khí sắc trầm là hay gặp nhất (100%); trong các triệu chứng phổ biến của trầm cảm, hay gặp nhất là giảm sút tính tự trọng, lòng tự tin và bi quan về tương lai với cùng tỷ lệ 94,3%. Kết luận: Trầm cảm là rối loạn tâm thần thường gặp sau chấn thương tuỷ sống. Trầm cảm thường khởi phát đầu tiên bởi buồn chán, bi quan. Triệu chứng đặc trưng hay gặp nhất là khí sắc trầm, triệu chứng phổ biến hay gặp là giảm sút tính tự trọng, lòng tự tin và bi quan về tương lai.

Depression is the most common mental disorder in patients with spinal cord injury. Depression affects many aspects such as reducing quality of life, ineffective exercise and recovery, increasing the risk of suicide, especially affecting the recovery process and social reintegration. Objectives: To describe clinical features of depressive disorders in patients with spinal cord injury. Subjects and research methods: A cross-sectional descriptive study of 107 inpatients with spinal cord injury who were treated at Rehabilitation Center, Bach Mai Hospital and Minimally Invasive Spine Department, Central Acupuncture Hospital f-rom August 2020 to August 2021. Results: The study subjects sociademographic c-haracteristics: men - 87.9%; average age - 43.25 ± 13.74; living in rural areas - 65.4%: lower than secondary education – 38,3%. Some main clinical features: 32.7% patients with depressive disorders, (18.7% mild level) according to ICD-10 criteria; the most common onset symptom was sadness, pessimistic thought (40%); all patients have depressed mood (100%); most commonly low self-esteem, self-confidence and pessimistic thought about the future with the same 94.3%. Conclusion: Depression was a common mental disorder in patients with spinal cord injury. Depression was often first triggered by sadness, pessimistic thought. The most typical symptom was a depressed mood, the most common symptom was low self-esteem, self-confidence and pessimistic thought about the future.
