Nghiên cứu mô tả một số đặc điểm lâm sàng và cận lâm sàng ở người bệnh nhiễm virus Epstein-Barr trong tủy xương. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang trên 71 người bệnh có kết quả xét nghiệm PCR-EBV dương tính trong dịch tủy xương tại Trung tâm huyết học - Truyền máu, Bệnh viện Bạch Mai từ tháng 1/2023 đến tháng 1/2024. Kết quả: Kết quả thường gặp nhiều ở người bệnh trên 60 tuổi và gặp nhiều hơn ở người suy giảm miễn dịch với các triệu chứng lâm sàng HLH (60,6%), sốt (59,2%), lách to 38% và viêm phổi 38%. Về tổng phân tích tế bào máu ngoại vi có đến 83,1% người bệnh thiếu máu, trong tủy xương thì 100% người bệnh có hiện tượng tăng đại thực bào, 74,6% có dấu hiệu thưc bào tế bào máu và 64,3% tăng lympho. Kết luận: Người bệnh nhiễm EBV gặp ở rất nhiều chuyên khoa, đặc biệt trên những bệnh nhân có suy giảm miễn dịch 19,7%, với triệu chứng lâm sàng thường gặp là sốt 59,2%, lách to (39,4%) và có thể các biến chứng nặng như viêm phổi (38%), (Hemophagocytic Lymphohistiocytosis-HLH) (60,6%).
To c-haracterize some clinical and subclinical features in patients with Epstein-Barr virus infection in the bone marrow. Subjects and methods: Cross-sectional descriptive study on 71 patients with positive PCR - EBV test results in bone marrow À uid at the Center for Hematology and Blood Transfusion, Bach Mai Hospital f-rom January 2023 to January 2024. Results: The results were more common in patients over 60 years of age and more common in immunocompromised patients with clinical symptoms of HLH (60.6%), fever (59.2%), splenomegaly (38%) and pneumonia 38%. In terms of total peripheral blood cell analysis, up to 83.1% of patients with anemia, in the bone marrow, 100% of patients had hyperphagocytosis, 74.6% showed signs of phagocytosis and 64.3% had hyperlymphocyte. Conclusions: Patients with EBV are found in many specialties, especially in patients with immunodeficiency of 19.7%, with common clinical symptoms of fever (59.2%), splenomegaly (39.4%) and possible severe complications such as pneumonia (38%), HLH (60.6%).
