Mô tả đặc điểm lâm sàng, cận lâm sàng và kết cục điều trị của viêm màng não ở bệnh nhân zona có tổn thương da.Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả loạt ca bệnh gồm 15 bệnh nhân được chẩn đoán viêm màng não do virus Varicella zoster (VZV) tại Khoa Da liễu và Bỏng, Bệnh viện Bạch Mai. Các biến số được thu thập bao gồm đặc điểm nhân khẩu học, biểu hiện lâm sàng, dịch não tủy, kết quả PCR VZV, hình ảnh cộng hưởng từ và kết cục điều trị.Kết quả: Tuổi trung vị của bệnh nhân là 59,9; 53,3% ≥60 tuổi; 46,7% là nam giới. Thời gian từ khởi phát phát ban đến xuất hiện triệu chứng thần kinh trung vị 4,9 ngày. Đau đầu gặp ở 100% trường hợp, trong khi hội chứng màng não điển hình ít gặp (cứng gáy 20%); không ghi nhận sốt, rối loạn tri giác hoặc co giật. Tổn thương zona khu trú vùng đầu–mặt–cổ ở tất cả bệnh nhân. Dịch não tủy có tăng tế bào mức vừa (trung vị 129,7 tb/mm³), ưu thế lympho (61,5%), protein tăng nhẹ (0,72 g/L), glucose bình thường. PCR VZV dương tính ở 86,7% trường hợp; không ghi nhận bất thường trên cộng hưởng từ não. Tất cả bệnh nhân được điều trị acyclovir tĩnh mạch (thời gian trung vị 11,5 ngày), 20% phối hợp corticosteroid. Kết cục thuận lợi với 100% hồi phục hoàn toàn, không ghi nhận di chứng hay tử vong.Kết luận: Viêm màng não do VZV ở bệnh nhân zona có tổn thương da thường biểu hiện lâm sàng không điển hình, chủ yếu là đau đầu đơn thuần và có thể thiếu các dấu hiệu màng não kinh điển. Xét nghiệm dịch não tủy và PCR VZV đóng vai trò then chốt trong chẩn đoán. Điều trị acyclovir sớm cho tiên lượng tốt.
To describe the clinical c-haracteristics, laboratory findings, and treatment outcomes of meningitis in patients with herpes zoster and cutaneous lesion. Materials and Methods: We conducted a descriptive case series of 15 patients diagnosed with Varicella zoster virus (VZV) meningitis at the Department of Dermatology and Burns, Bach Mai Hospital. Demographic data, clinical manifestations, cerebrospinal fluid (CSF) findings, VZV PCR results, magnetic resonance imaging (MRI) findings, and treatment outcomes were analyzed. Results: The median age was 59.9 years; 53.3% were aged ≥60 years and 46.7% were male. The median interval f-rom rash onset to neurological symptom development was 4.9 days. Headache was present in all patients (100%), whe-reas classical meningeal signs were uncommon (neck stiffness in 20%); no cases of fever, al-tered consciousness, or seizures were observed. Cutaneous lesions were localized to the head–face–neck region in all cases. CSF analysis demonstrated moderate pleocytosis (median 129.7 cells/mm³) with lymphocytic predominance (61.5%), mildly elevated protein (0.72 g/L), and normal glucose levels. VZV PCR was positive in 86.7% of patients. No abnormal findings were detected on brain MRI. All patients received intravenous acyclovir for a median duration of 11.5 days; adjunctive corticosteroids were administered in 20% of cases. Clinical outcomes were favorable, with complete recovery in all patients and no neurological sequelae or mortality. Conclusion: VZV meningitis in patients with herpes zoster and cutaneous lesions often presents with atypical clinical features, predominantly isolated headache without classical meningeal signs. CSF analysis and VZV PCR are essential for diagnosis. Early initiation of intravenous acyclovir is associated with an excellent prognosis.
