Đánh giá đặc điểm lâm sàng và kết quả phẫu thuật nội soi đường miệng trong điều trị ung thư tuyến giáp xâm lấn cơ trước giáp. Đối tượng và phương pháp nghiên cứu:Tiến cứu trên 28 bệnh nhân ung thư tuyến giáp thể nhú xâm lấn cơ trước giáp( T3b) được phẫu thuật nội soi tại Bệnh viện Đại học Y Hà Nội từ 1/2019 đến tháng 12/2021. Kết quả: Độ tuổi trung bình là 39±8,9; 92,8% nữ giới. Trên siêu âm kích thước u trung bình là 8,3±3,2mm, không có trường hợp nào phát hiện u xâm lấn cơ trước giáp trên siêu âm trước mổ. Tất cả trường hợp sau mổ chẩn đoán ung thư tuyến giáp thể nhú, có xâm nhập mô cơ vân, 10 trường hợp (35,7%) phát hiện di căn hạch cổ trung tâm sau mổ. Thời gian phẫu thuật trung bình là 120±24 phút. Nói khàn tạm thời gặp ở 7,1% trường hợp, tê bì tay chân gặp ở 3,6% trường hợp. Nồng độ Tg, anti-Tg cao sau mổ gặp ở 8 trường hợp (28,5%), tuy nhiênnồng độ thấp dưới ngưỡng sau khi điều trị I131. Kết luận: Phẫu thuật nội soi đường miệng đạt được an toàn, hiệu quả và bước đầu đảm bảo về mặt ung thư học trong điều trị ung thư tuyến giáp xâm lấn cơ trước giáp.

To evaluatethe clinical characteristicsand the results of transoral endoscopic thyroidectomy vestibular approach (TOETVA) in the treatment of thyroid cancer with invasion of the strap muscle. Methods: A prospective study on 28 patients with papillary thyroid cancer invading stap muscle by TOETVA at Hanoi Medical University Hospital from January 2019 to December 2021. Results: The mean age was 39±8.9; 92.8% female. Results of ultrasound presented that the average tumor size was 8.3±3.2mm, there was no case of tumor invading the strap muscle on preoperative ultrasound. Histopathology after surgery was 100% papillary thyroid cancer, with invasion strap muscle. Metastasis of the central lymph nodes was detected in 10 cases (35.7%). The average surgery time was 120±24 minutes. Temporary hoarseness was found in 7.1% of cases, hypocalcemia was found in 3.6% of cases. High levels of Tg or anti-Tg after surgery were found in 8 cases (28.5%), however theconcentration of Tg and anti-Tg were below thenormal value after I131 treatment. Conclusion:TOETVA still achieves safety, efficacy and initial oncological assurance in the treatment of thyroid cancer with invasion of the strap muscle.
