To evaluate the efficacy and safety profile of transoral endoscopic thyroidectomy vestibular approach (TOETVA) procedure in patients with differentiated thyroid cancer. Patients and method: Prospective description study of 865 patients with with differentiated thyroid cancer who received TOETVA surgery at Hanoi Medical University hospital from 6/2018 to 9/2022. Results: The mean age is 36,1 ± 9,1, 96% female. Total thyroidectomy: 137 cases (15,9%); lobectomy and isthmusectomy: 728 cases (84,1%). Central neck dissection was perfomed in all cases: unilateral CND in 745 case (86,1%), bilateral CND in 120 cases (13,9%), The number of harvested and metastatic lymph node is 5,98 ± 4,1 nodes and 1,04 ± 1,7 nodes, respectively. 39,9% patients had lymph node metastatic in post-operative pathology results. Transitent hypoparathyroidism: 27 cases (3,1%); transitent vocal cord palsy: 24 cases (2,7%); numb chin: 10 patients (1,1%). There were no cases of permanent hypoparathyroidism, permanent vocal cord palsy and numb chin. Infection complications did not occur in our patients. 255 (29,5%) case were satisfied, 609 (70,5%) were extremely satisfied with the outcomes of operations.With median follow-up time was 30 (1 - 51) months, local and regional recurrence was detected in 3 patients (0,3%). Conclusions: With appropriate indications, TOETVA surgery in differentiated thyroid cancer brings high efficiency in oncology aspects and has outstanding advantages in terms of aesthetics.
