Đánh giá đặc điểm Alanine aminotransferase (ALT), Aspartate transferase (AST) và Bilirubin toàn phần ở bệnh nhân (BN) COVID-19 mức độ nặng và nguy kịch điều trị tại Bệnh viện dã chiến truyền nhiễm 5G. Đối tượng và phương pháp nghiên cứu: Nghiên cứu thuần tập, hồi cứu trên 156 BN COVID-19 mức độ nặng và nguy kịch điều trị tại Khoa Hồi sức cấp cứu -Bệnh viện Dã chiến Truyền nhiễm 5G- Thành phố Hồ Chí Minh từ tháng 8 đến tháng 11/2021. Kết quả: 48,7% BN có tổn thương gan (ALT >40 U/l), trong đó chủ yếu ở mức độ nhẹ (ALT trong khoảng 41- 79 U/l). Định lượng bilirubin toàn phần, ALT và AST có xu hướng tăng dần từ nhóm bình thường, nhóm tổn thương gan mức độ nhẹ, mức độ vừa đến mức độ nặng, khác biệt có ý nghĩa thống kê. Thời gian sống trung bình của nhóm BN không có tổn thương gan là 17,96 ± 1,57 ngày cao hơn ở nhóm có tổn thương gan là 11,89 ± 0,89 ngày, khác biệt có ý nghĩa thống kê với p=0,0073. Kết luận: BN COVID- 19 mức độ nặng và nguy kịch trong nhóm nghiên cứu phần lớn có tổn thương gan mức độ nhẹ (ALT trong khoảng 41- 79 U/l).Tổn thương gan càng nặng thì nồng độ của ALT, AST và bilirubin toàn phần càng tăng cao.Tuy nhiên, chỉ có sự thay đổi của ALT và AST có ý nghĩa lâm sàng, còn sự thay đổi của Billirubin toàn phần chỉ có ý nghĩa thống kê. Thời gian sống trung bình nhóm không tổn thương gan cao hơn nhóm tổn thương gan.

To evaluate the characteristics of Alanine aminotransferase (ALT), Aspartate transferase (AST) and Total bilirubin in severe and critical COVID-19 patients, who were treated in Infectious Field Hospital No.5G. Subjects and methods: A respective cohort study on 156 severe and critical COVID-19 patients in ICU of Infectious Field Hospital No.5G- Hochiminh city from August to November 2021. Results: 48.7% of patients had a liver damage (ALT >40 U/l), almost at a mild level (ALT ranged from 41-79UI).Concentration of total bilirubin,ALT and AST tended to increase gradually from the normal group, the group with mild liver damage, moderate to severe liver damage; the difference was statistically significant. The mean survival time of the group without liver damage (17.96 ± 1.57 days) was higher than the group with liver damage (11.89 ± 0.89 days); the difference was statistically significant with p = 0.0073. Conclusion: Severe and critical COVID-19 patients in the study mostly had mild liver damage (ALT ranged from 41- 79 U/l). The more serve the liver damaged, the higher the concentration of ALT, AST and total bilirubin increased. However, only the change of ALT and AST was clinically significant, while the change of total bilirubin was only statistically significant. The mean survival time in the group without liver damage was higher than in the group with liver damage.
