EDTA đã được khuyến cáo là chất chống đông máu được lựa chọn để thực hiện các xét nghiệm huyết học vì nó cho phép bảo quản tốt nhất các thành phần tế bào và hình thái của tế bào máu. Tuy nhiên, EDTA có thể gây ra sự kết tụ tiểu cầu dẫn đến số lượng tiểu cầu thấp trên máy phân tích huyết học tự động. Hiện tượng này được gọi là giảm tiểu cầu giả phụ thuộc EDTA (EDTA-dependent pseudothrombocytopenia). Chúng tôi báo cáo một trường hợp giảm tiểu cầu giả phụ thuộc EDTA (EDTA-PTCP) ở một bệnh nhân được chẩn đoán viêm phổi COVID 19 tại Bệnh viện Dã chiến trường Đại học Kỹ thuật Y tế Hải Dương. Tại thời điểm nhập viện bệnh nhân không có dấu hiệu chảy máu, xét nghiệm số lượng tiểu cầu bình thường, bệnh nhân được điều trị theo phác đồ có sử dụng heparin trọng lượng phân tử thấp (enoxaparine). Sau điều trị 5 ngày số lượng tiểu cầu giảm dần, chúng tôi nghi ngờ nguyên nhân giảm tiểu cầu này là do hepanrin nên dừng sử dụng. Tuy nhiên số lượng tiểu cầu sau đó có xu hướng tăng chậm và cũng không có bất cứ dấu hiệu chảy máu hay huyết khối nào ở bệnh nhân. Chúng tôi phết lam máu ngoại vi để đánh giá thì phát hiện có các đám kết tụ tiểu cầu, nên tiến hành xét nghiệm mẫu máu sử dụng chất chống đông khác EDTA là citrat được ủ ấm ở nhiệt 37 độ C, kết quả số lượng tiểu cầu ở giới hạn bình thường với tất cả ngày sau đó. Như vậy giảm tiểu cầu giả phụ thuộc EDTA không có ý nghĩa bệnh lý và phết lam máu ngoại vi với tất cả các trường hợp giảm tiểu cầu là vô cùng cần thiết, từ đó đưa ra các biện pháp để xác định chính xác số lượng tiểu cầu. Từ ca lâm sàng chúng tôi cũng nhận thấy có thể ở bệnh nhân COVID19 xuất hiện các kháng thể gây phản ứng với kháng nguyên tiểu cầu bị thay đổi bởi EDTA gây nên hiện tượng giảm tiểu cầu giả.

EDTA has been recommended as the anticoagulant of choice for hematological testing because it allows the best preservation of cellular components and morphology of blood cells. However, in EDTA induced platelet clumping and platelet agglutination result in low platelet count on automated hematology analyzers. The phenomenon was called EDTA dependent pseudo-thrombocytopenia (EDTA-PTCP) We report a case of EDTA-PTCP in a pneumonia due to 2019 novel coronavirus disease (COVID- 19). A 62 year old male patient was diagnosed with severe pneumonia due to COVID 19. At the presentation, he did not have any signs of bleeding, and the platelet count was normal, he was initially treated with enoxaparin. After 5 days, his platelet counts gradually decreased, heparin induced thrombocytopenia was a suspected diagnosis, so enoxaparin stopped immediately. However, the platelet counts slowly increased and the patient did not have any signs of bleeding.The peripheral blood smear was performed, it showed a platelet aggregation. The whole blood sample was recollected in tube used sodium citrate as the anticoagulant and warmed up at 37C before being tested by Automatic Blood Analyzer Machine , result showed number of platelets was normal. Thus, EDTA-dependent pseudothrombocytopenia has no clinical significance and a peripheral blood smear in all cases of thrombocytopenia is essential, thereby providing measures to accurately determine the amount of thrombocytopenia. . From this case report, it is possible that in COVID19 patients, antibodies that react with platelet antigens are altered by EDTA, causing the EDTA - pseudothrombocytopenia.
