Mô tả đặc điểm của chỉ số Fib-4 của bệnh nhân được chẩn đoán xơ hóa gan tại bệnh viện Việt Tiệp (6/2021). Khảo sát giá trị chỉ số Fib-4 và kết quả FibroScan trong đánh giá tình trạng xơ hóa gan tại điểm nghiên cứu. Đối tượng và phương pháp nghiên cứu: bệnh nhân được chẩn đoán xơ gan đến khám và điều trị tại bệnh viện Việt Tiệp, có chỉ định làm FibroScan; tiểu cầu, AST, ALT; nghiên cứu mô tả cắt ngang Kết quả: 170 bệnh nhân xơ gan, tuổi trung bình 48,59 ± 14,40, 64,1% (109) là nam giới với. Chỉ số Fib-4 tăng dần theo tình trạng xơ hóa gan từ 1,14 ± 0,53 ở F0-F1 đến 5,11 ± 3,14 ở F4 và có mối tương quan với các mức độ xơ hóa gan (r=0,605; p<0,001). So với kết quả FibroScan: dưới điểm cắt 1,45: Fib-4 có độ nhạy 81% và giá trị dự đoán âm tính 91,8%. Trên điểm cắt 3,25: Fib-4 có độ đặc hiệu 100%, giá trị dự đoán dương tính 100% và giá trị dự đoán âm tính 88,3%. 93,45% các trường hợp nằm ngoài khoảng 1,45 – 3,25 được Fib-4 phân loại tình trạng xơ hóa gan một cách chính xác (69,49%). Kết luận: Fib-4 có tương quan với tình trạng xơ hóa gan; có giá trị lớn trong phân biệt xơ hóa nhẹ và xơ hóa nặng. Phần lớn bệnh nhân có Fib-4 ngoài khoảng 1,45 – 3,25 có thể không phải thực hiện xét nghiệm FibroScan.

Investigate the value of fib-4 and Fibroscan test in the assessment of cirhosis Subjects and Methods: patients were diagnosed cirhosis who came to Viettiep hospital with FibroScan test, platelet count, AST, and ALT (6/2021). Results: there are 170 patients diagnosed with cirhosis, average age of 48,59 ± 14,40, 64.1% were male. FIB-4 index increased with the severity of liver fibrosis from 1,14 ± 0,53 at F0-F1 to 5,11 ± 3,14 at F4 and had correlation with fibrosis stages (r=0.605; p<0.001). Compared to FibroScan, FIB-4 can distinguish slight and severe liver fibrosis: under cut-off value of 1.45: 81% sensivity and negative predictive value of 91.8%; over cut-off value 3.25: 100% speciality, positive predictive value of 100% and negative predictive value of 88.3%. Moreover, 93.45% patients with FIB-4 index outside the rage 1.45 – 3.25 had their liver fibrosis condition properly categorized (69.49% of entire sample size). Conclusion: FIB-4 index was correlated to liver fibrosis stages and showed great value in distinguishing slight and severe liver fibrosis. Most patients with FIB-4 index out of the 1,45 – 3,25 range can be considered to not use FibroScan.
