Khảo sát nồng độ TREC ở trẻ sơ sinh và phát hiện trẻ có nguy cơ cao mắc bệnh SCID tại Bệnh viện Phụ Sản Hà Nội. Đối tượng: Gồm 47620 trẻ được làm xét nghiệm TREC, trong đó có 12 trẻ có nồng độ TREC ở ngưỡng nguy cơ cao. Phương pháp: Nghiên cứu mô tả một loạt ca bệnh kết hợp hồi cứu và tiến cứu. Kết quả: Nồng độ TREC trung vị 104,0cp/μL. Nồng độ TREC trung vị ở trẻ nam là 98,3cp/μL và nữ là 111,0cp/μL. Nồng độ TREC có xu hướng tăng theo tuần thai, cao nhất ở nhóm từ 28 đến dưới 32 tuần thai: giá trị trung vị là 113,0cp/μL. Tám trong tổng số 12 trẻ có kết quả sàng lọc TREC < 24cp/μL, đến khám tại Bệnh viện Nhi Trung ương ghi nhận có 2 bệnh nhân được chẩn đoán xác định bệnh SCID bằng xét nghiệm phân tích gene, tương đương với tỷ lệ SCID trong quần thể nghiên cứu 4,2/100000 trẻ sinh sống. Kết luận: Sàng lọc SCID bằng phương pháp xét nghiệm định lượng TREC là phương pháp hiệu quả, giúp phát hiện sớm những người bệnh nguy cơ cao, cần được thực hiện rộng rãi.

To investigate the concentration of TREC in newborns and detect children at high risk of SCID at Hanoi Obstetrics and Gynecology Hospital. Subjective: 47620 children were tested for TREC, of which 12 children had high-risk TREC levels. Methods: A descriptive study with a combination of retrospective and prospective case series. Results: Median TREC concentration 104,0cp/μL. The median TREC concentration in boys was 98,3cp/μL and in girls 111,0cp/μL. TREC concentration tends to increase with gestational weeks, the highest in the group from 28 to less than 32 weeks of gestation: the median value is 113,0cp/μL. Eight out of 12 children with TREC screening results < 24cp/μL, who came to the Vietnam National Children’s Hospital, 2 patients were diagnosed with SCID by genetic analysis, equivalent to the rate SCID in the study population 4,2/100000 live births. Conclusion: SCID screening by quantitative TREC test method is an effective method, helping to detect high-risk patients early, and should be widely implemented.
