Ghép thận, Người hiến chết

Trình bày kết quả trì hoãn chức năng thận ghép (Delay graft function, DGF), và các yếu tố ảnh hưởng đến DGF. Phương pháp nghiên cứu: Nghiên cứu mô tả hàng loạt ca, hồi cứu, bao gồm những trường hợp (TH) DDKTx, từ tháng 4/2008 đến tháng 12/ 2021, tại bệnh viện Chợ Rẫy. Kết quả: Nghiên cứu có 51 trường hợp (TH) DDKTx, tuổi trung vị 37. Nam giới chiếm 72,5%. Trung vị chỉ số khối cơ thể 20,9. Trung vị thời gian điều trị thay thế thận 39,3 tháng, thận nhân tạo là phương pháp đều trị chủ yếu chiếm 88,2%. Trung vị thời gian theo dõi là 5 năm. DGF gặp trong 12 TH, chiếm 23,5%. Các yếu tố nguy cơ làm tăng DGF gồm có: thận từ người hiến chết tuần hoàn, người hiến thận có tuổi trên 60, mức độ bất tương hợp HLA trên 3, không tương hợp HLD-DR, thận hiến có chỉ số KDPI trên 80%, thời gian thiếu máu lạnh trên 12 giờ. Kết luận: Tỷ lệ DGF của DDKTx tại bệnh viện Chợ Rẫy tương tự các trung tâm ghép khác trên thế giới, khẳng định đây là nguồn tạng hiến quan trọng để phát triển trong tương lai, góp phần giả quyết vấn đề thiếu hụt nguồn thận hiến ở nước ta.
