Nghiên cứu được thực hiện để đánh giá kết quả phẫu thuật bệnh động mạch vành (ĐMV) trái xuất phát bất thường từ động mạch phổi (ĐMP) trên 37 bệnh nhân mắc được phẫu thuật tại Bệnh viện Nhi Trung ương. Tất cả 37 bệnh nhân được phẫu thuật theo phương pháp trồng lại gốc ĐMV trái, 2 bệnh nhân sửa van hai lá cùng thì với trồng lại ĐMV. Có 2 bệnh nhân tử vong sớm (trong 30 ngày đầu) chiếm 5,4%, không có bệnh nhân tử vong muộn. Biến chứng sau phẫu thuật thường gặp là hội chứng cung lượng tim thấp và nhiễm trùng. Tỷ lệ bệnh nhân không có suy tim (Ross 1) tăng từ 7% (trước phẫu thuật) lên 75% (12 tháng sau phẫu thuật) (p < 0,05). Phân suất tống máu thất trái (LVEF) trung bình trước phẫu thuật là 39,8 ± 17,8% đã tăng lên 70,1 ± 8,6% lúc 12 tháng sau phẫu thuật (p < 0,05). Đường kính cuối tâm trương thất trái (LVDd) trung bình giảm từ 9,8 ± 4,0 Z-score trước phẫu thuật xuống còn 2,3 ± 1,6 Z-score lúc 12 tháng sau phẫu thuật (p < 0,05). Tỷ lệ bệnh nhân hở van hai lá (HoHL) đã giảm từ 89% (trước phẫu thuật) xuống 75% (12 tháng sau phẫu thuật) (p < 0,05). Phẫu thuật bệnh ĐMV trái xuất phát từ ĐMP có tỷ lệ tử vong, mức độ suy tim trên lâm sàng, LVEF, LVDd đều cải thiện đáng kể, mức độ HoHL cũng giảm tuy nhiên chưa có nhiều sự cải thiện rõ rệt.

A review of the Cross - Sectional descriptive studies of 37 patients at the National Children's Hospital was conducted. All patients was operated by the left coronary button transferring method to repair the left coronary artery from the pulmonary artery (ALCAPA); two patients underwent mitral valve repair concurrent with ALCAPA repair. 2 patients passed away within the first 30 days (5,4%), no one passed away in later date. Common post surgery complications are low cardiac output and infection. Heart failure rate (Ross 1) increased from 7% (before surgery) to 75% (12 months after surgery) (p < 0.05). The average of left ventricular ejection fraction (LVEF) improved from 39.8 ± 17.8% to 70.1 ± 8.6% in 12 months after surgery (p < 0.05). The average of left ventricular end-diastolic diameter (LVDd) decreased from 9.8 ± 4.0 Z-score pre-surgery to 2.3 ± 1.6 Z-score 12 months post (p < 0.05). Mitral regurgitation (MR) rate fall from 89% (pre-surgery) to 75% (12-month-post-surgery) (p < 0.05). Patients underwent ALCAPA repair heart failure rate was above clinical; both LVEF and LVDd had significant improvement, MR rate also decreased although there were not many visible developments.
