Hạ canxi máu là rối loạn không hiếm gặp ở trẻ em đặc biệt trẻ dưới 2 tuổi. Biểu hiện lâm sàng đa dạng từ không có triệu chứng, co giật, li bì, bú kém, thậm chí có các biểu hiện nặng như bệnh cảnh sốc tim và rối loạn nhịp tim. Mục tiêu: mô tả đặc điểm lâm sàng và hóa sinh của hạ canxi máu ở trẻ dưới 2 tuổi. Đối tượng và phương pháp: 20 trẻ dưới 2 tuổi được chẩn đoán hạ canxi máu điều trị nội trú tại Bệnh viện Nhi Trung ương trong 18 tháng (10/2019 - 4/2021), nghiên cứu mô tả một loạt ca bệnh. Kết quả: tuổi xuất hiện triệu chứng phổ biến nhất làdưới 2 tháng (80%). Các triệu chứng lâm sàng bao gồm co giật (95%), kích thích – quấy khóc nhiều (45%), li bì(10%) và 2 trẻ có rối loạn nhịp tim Qtc kéo dài.Nồng độ canxi máu giảm nặng với mức canxi toàn phần là 1,32±0,22 mmol/l, canxi ion là 0,7±0,15 mmol/l, nồng độ vitamin D máu trung bình giảm (21,25± 15,85 nmol/l). Kết luận:Tỷ lệ cao hạ canxi máu gặp ở trẻ dưới 2 tháng tuổi với triệu chứng lâm sàng chính là co giật. Bổ sung vitamin D cho trẻ ngay sau sinh, bổ sung chế độ dinh dưỡng hợp lý giàu canxi và vitamin D cho mẹ trong suốt quá trình mang thai và cho con bú giúp giảm nguy cơ hạ canxi ở trẻ dưới 2 tuổi.

Hypocalcemia is common emergency disorder in children, especially children less than 2 years of age. Diversity of clinical manifestations are f-rom asymptomatic, convulsions, lethargy, poor feeding, and severe cases hospitalized with cardiogenic shock, arrhythmia. Objectives: to describe the clinical and biochemistry c-haracteristics of hypocalcaemia in children less than 2 years of age. Patients and methods: 20 children less than 2 years of age were diagnosed with hypocalcemia treated at the National Children's Hospital during 18 months (Oct 2019 to Apr 2021). This is descriptive case series study. Results: The disorder is most common in children less than 2 months of age (80%) with symptoms of convulsions (95%), irritation - crying, lethargy, in addition, two cases presented with cardiac arrhythmia extended Qtc. Blood calcium levels were severely reduced with total calcium levels 1.32±0.22 mmol/l, ion calcium levels 0.7±0.15 mmol/l. Vitamin D deficiency was proved in both child and the child's mother. Conclusion: Hypocalcemia is common in children less than 2 months of age. The main clinical symptom was seizures. Supplementing vitamin D for children right after birth, supplementing with a reasonable diet rich in calcium and vitamin D for mothers during pregnancy and lactation helps to reduce the risk of hypocalcemia in children less than 2 years of age.
