Mô tả các đặc điểm lâm sàng, điện não đồ, hình ảnh não và các biến dị di truyền ghi nhận được trên trẻ mắc hội chứng West. Phương pháp: Hồi cứu hồ sơ bệnh án hội chứng West tại Bệnh viện Nhi Đồng 2 giai đoạn 2019 – 2021, thu thập các dữ liệu lâm sàng, cận lâm sàng và phân tích biến dị di truyền bằng phần mềm VarSome, đối chiếu công cụ OMIM và y văn để tìm hiểu tương quan giữa lâm sàng và di truyền. Kết quả: Khảo sát 20 bệnh nhi mắc hội chứng West (13 nam, 7 nữ, 70% là con đầu). Trung vị tuổi khởi phát cơn co thắt là 5,3 tháng. 50% thay đổi điện não không điển hình. 55% có bất thường trên CT/MRI não. 65% bệnh nhi có bất thường về phát triển tâm vận. 17/20 trường hợp ghi nhận có biến dị di truyền, trong đó có 26 biến dị có tiềm năng sinh bệnh của 21 gen với 9/17 trẻ có 1 biến dị và 1/17 có tới 3 biến dị. 62% biến dị thuộc phân loại “chưa rõ khả năng sinh bệnh”. 11/26 là đột biến sai nghĩa, 11/26 là đột biến vùng không mã hóa. 5/20 trường hợp với 8 gen có khả năng liên quan đến hội chứng West gồm: CREBBP, MED25, HDAC8, SCN3A, ABCD1, TSC2, COL4A1, NDUFA10. 12/20 trường hợp với 13 gen chưa được xác nhận liên quan hội chứng West gồm: STXBP1, ST3GAL3, AKT3, SCN8A, CHD2, GABBR2, GRIN2B, PRODH, GABRB3, TUBB2A, POLG, CYFIP2, CACNA1A. Kết luận: Việc xác định tương quan giữa đặc điểm lâm sàng và di truyền của hội chứng West vẫn còn là thách thức lớn.

To describe the clinical features, electroencephalography, brain imaging and genetic variations recorded in children with West syndrome. Methods: Retrospective study of West syndrome at Children's Hospital 2 for the period of 2019 – 2021, collecting clinical, investigative and genetic variation analysis by VarSome software, comparing OMIM and literature to find out the correlation between phenotype and genotype. Results: 20 childrens with West syndrome were surveyed (13 males, 7 females, 70% being the first child). The median age of spasm onset was 5.3 months. 50% of EEG changes are atypical. 55% had abnormalities on brain CT/MRI. 65% of children have abnormalities in psychomotor development. 17/20 cases were recorded with genetic variation, of which there were 26 potential pathogenic variants of 21 genes with 9/17 children having 1 mutation and 1/17 having up to 3 mutations. 62% of mutations were classified as "pathogenicity unknown". 11/26 is a false meaning mutation, 11/26 is a non-coding region mutation. 5/20 cases with 8 genes potentially related to West syndrome include: CREBBP, MED25, HDAC8, SCN3A, ABCD1, TSC2, COL4A1, NDUFA10. 12/20 cases with 13 unconfirmed genes associated with West syndrome include: STXBP1, ST3GAL3, AKT3, SCN8A, CHD2, GABBR2, GRIN2B, PRODH, GABRB3, TUBB2A, POLG, CYFIP2, CACNA1A. Conclusion: Determining the correlation between clinical and genetic characteristics of West syndrome remains a major challenge.
