U sợi thần kinh type 1 (neurofibromatosis type 1 - NF1) là bệnh di truyền trội trên nhiễm sắc thể (NST) thường do đột biến gen neurofibromin (NF1) gây ra. Bệnh có biểu hiện đặc trưng là các khối u ở mô thần kinh, thường gặp trên da. Nghiên cứu với mục tiêu mô tả đặc điểm lâm sàng, gia hệ, kết quả xét nghiệm gen NF1 và tư vấn di truyền của ba ca bệnh NF1. Nghiên cứu qua khai thác phả hệ, tiền sử, bệnh sử, thăm khám lâm sàng sơ bộ. Đột biến gen NF1 được phát hiện bằng xét nghiệm giải trình tự hệ thế hệ mới và/ hoặc xét nghiệm giải trình tự trực tiếp Sanger. Các đặc điểm lâm sàng rất thay đổi giữa các bệnh nhân mắc NF1, bệnh nhân có thể có tiền sử gia đình có người bị bệnh hoặc không. Các ca bệnh có đột biến gen khác nhau trên gen NF1 và đều dẫn tới ngừng dịch mã. Tư vấn di truyền nhằm cung cấp các thông tin liên quan đến tiến triển và biến chứng có thể gặp phải của bệnh, các biện pháp điều trị bệnh hiện tại, quy luật di truyền, nguồn gốc đột biến và khả năng di truyền gen bệnh cho con cái. Xác định đột biến gây bệnh có ý nghĩa quan trọng cho chẩn đoán sớm và chẩn đoán trước sinh/chẩn đoán trước chuyển phôi.

Neurofibromatosis type 1 is an autosomal dominant disease caused by mutations of the neurofibromin gene (NF1). Its clinical manifestations include neurofibroma tumors, typically on the skin. This article aimed to report three NF1 cases with different characteristics on symptoms, pedigree, mutation analysis, and genetic counseling. Our method included the investigation of pedigree, historical medical record and clinical examination. NF1 mutations were tested by next generation sequencing and/or Sanger sequencing. Phenotypes of NF1 patients were extremely variable, with or without a positive family history of the disease. Genotypes of these cases were unique but all leading to stop gained. Genetic counseling provided information on the progression and possible complications of this disorder, current treatment therapies, mode of inheritance, origins of mutations, the risk of inheriting a mutation to offspring. Molecular genetic testing for the identification of a pathogenic variant is essential for early diagnosis, prenatal or preimplantation genetic testing.
