Mô tả các đặc điểm lâm sàng, cận lâm sàng và điều trị ở bệnh nhi được chẩn đoán VNTM kháng LGI1 . Phương pháp: Nghiên cứu hồi cứu 58 bệnh nhân dưới 16 tuổi được chẩn đoán VNTM từ năm 2019 đến năm 2022 tại khoa Thần kinh bệnh viện Nhi Đồng 2. Triệu chứng lâm sàng, cận lâm sàng, và kết cục điều trị được ghi lại, so sánh giữa hai nhóm VNTM kháng LGI1 (LGI1 (+)) và VNTM kháng thể kháng LGI1 âm tính (LGI1 (-)) Kết quả: VNTM tự kháng thể kháng thụ thể NMDA (N-methyl-D-aspartate) và nhóm có kháng thể âm tính cùng chiếm tỉ lệ cao nhất 43,1%, VNTM kháng LGI1 10,3% và VNTM kháng thụ thể GABA 3,45%. Triệu chứng lâm sàng không có khác biệt giữa hai nhóm. 80-90% bệnh nhân có đáp ứng với liệu pháp miễn dịch, nhóm viêm não kháng LGI1 có số bệnh nhân diễn tiến nặng cao hơn so với nhóm LGI1 (-). Có 1 bệnh nhân tử vong, chiếm tỉ lệ 1,7%. Tỉ lệ tái phát ở nhóm LGI1 dương tính (LGI1 (+)) cao hơn so với nhóm còn lại lần lượt là 66,7% với 5,77% (p < 0.001). Tỉ lệ bệnh nhân phải dùng thuốc chống động kinh ở nhóm LGI1 (+) cao hơn, (66,7% so với 26,9%), và tất cả bệnh nhân này đều được điều trị với ít nhất 2 thuốc, p= 0,008. Kết luận: Trẻ mắc VNTM kháng LGI1 ít gặp và ít được nhận diện. Tiên lượng ở trẻ thường tốt với đáp ứng liệu pháp miễn dịch, mặc dù có tái phát và ghi nhận tử vong. Nhận biết VNTM dựa vào triệu chứng lâm sàng kết hợp với phương pháp xét nghiệm kháng thể là chìa khóa để chẩn đoán và quyết định điều trị sớm, đem lại kết cục tốt cho bệnh nhân.

To investigate the clinical, paraclinical features and treatment outcomes of pediatric anti-LGI1 (Leucine-rich glioma-inactivated 1) encephalitis. Methods: We retrospectively analyzed 58 patients under 16 years of age diagnosed AE from 2019 to 2022 at Neurology Department of Children Hospital 2. Clinical features, laboratory or radiological findings and treatment outcomes were compared between anti-LGI1 encephalitis (LGI1 (+) and non anti-LGI1 encephalitis (LGI1 (-)). Results: Anti-NMDA receptor (N-methyl-D-aspartate) encephalitis and antibody-negative AE together accounted for the highest rate 43.1%, followed by anti-LGI1 encephalitis 10.3% and finally anti GABA receptor encephalitis 3.45%. Clinical characteristics were not different between the two groups. 80-90% of patients responded to immunotherapy, the number of patients with anti-LGI1 encephalitis having poor outcome is higher than that of the LGI1 group (-). There is 1 patient died, accounting for 1.7%. The recurrence rate in the LGI1-positive group (LGI1 (+)) was higher than that in the other group, 66.7% and 5.77%, respectively (p < 0.001). The proportion of patients requiring antiepileptic drugs was higher in the LGI1 (+) group, (66.7% vs 26.9%), and all these patients were treated with at least 2 drugs, p= 0.008. Conclusion: Pediatric anti-LGI1 encephalitis are rare and under-recognized. Prognosis in children is generally good with high response to immunotherapy, although recurrence and mortality have been reported. Recognition based on clinical features combined with antibody testing methods is the key to early diagnosis and treatment decisions, bringing good outcomes for patients.
