Đánh giá hiệu quả tăng cường giảm đau trong mổ, giảm đau sau mổ của phương pháp truyền liên tục thuốc tê vào mặt dưới cơ dựng sống 1 bên trên bệnh nhân tim ít xâm lấn (MICS) có nội soi đường ngực phải. Đối tượng và phương pháp: 30 BN (bệnh nhân) tuổi 18 – 75, mổ phiên MICS qua đường ngực phải có hỗ trợ nội soi tại Viện Tim mạch Việt Nam, Bệnh viện Bạch mai từ tháng tháng 11 năm 2018 đến tháng 7 năm 2020, đã được giảm đau bằng kĩ thuật ESPB bên phải. Catheter ESPB thực hiện dưới hướng dẫn siêu âm, đặt sau khi gây mê để phẫu thuật, tiêm ropivacaine 0,5%; 20ml. Khi kết thúc phẫu thuật: BN được truyền ropivacain 0,1% qua catheter ESP liên tục 0,2ml/kg/giờ ; kết hợp với paracetamol truyền tĩnh mạch 1g / 6h; BN được đánh giá điểm visual analogue scale (VAS) khi nghỉ, khi vận động tại các thời điểm trong vòng 72 giờ sau rút nội khí quản, lượng opioid dùng thêm, các tác dụng phụ. Kết quả: Hiệu quả tăng cường giảm đau trong mổ tốt, lượng fentanyl tiêu thụ 212,51 ± 71,58 μg; điểm VAS trung bình khi nghỉ, khi vận động < 4, có 5 BN (16,7%) cần dùng thêm PCA morphine tĩnh mạch, với lượng dùng thêm: trong 24h, 48h, 72h lần lượt: 4mg, 8mg, 14,2mg. Trong nghiên cứu không gặp các biến chứng nặng liên quan đến ESPB. Kết luận: Phương pháp ESPB có hiệu quả tăng cường giảm đau trong mổ, giảm đau sau mổ tốt mà không gặp các biến chứng nặng liên quan đến phương pháp ESPB

To assess the safety and efficacy of the analgesic of unilateral erector spinae plane block (ESPB) in minimally invasive cardiac surgery (MICS). From November 2018 to July 2020, Subjects nad method: 30 patients aged 18-75, underwent minimally invasive cardiac surgery via the right thoracotomy with endoscopic support at the Vietnam National Heart Institute, Bach Mai Hospital. Ultrasound-guided right unilateral ESP catheter performed after general anesthesia for surgery, injected ropivacaine 0.5%; 20ml. At the end of surgery: patient received continuously ropivacaine 0.1% through the ESP catheter 0.2ml/kg/hour; combined with paracetamol intravenous infusion 1g every 6 hours; Patients were assessed for visual analogue scale (VAS) score at rest, during exercise at times within 72 hours after extubation, additional opioid consumption, and adverse events. Results: Effective in enhancing pain relief during surgery, the amount of fentanyl consumed was 212.51 ± 71.58 μg. Postoperative period the average VAS score at rest, and dynamic < 4; 5 patients (16.7%) needed additional intravenous PCA morphine, with the amount consumption in 24h, 48h, 72h, respectively: 4mg, 8mg, 14.2mg. In the study, there were no serious complications related to ESPB. Conclusion: ESPB provided effective analgesic for MICS via thoracic incision in reducing the VAS score and the morphine required. It is also a safe method with no severe ESPB-related complications.
